today we will talk about bad breath this is a very common problem as you may have known I'll talk about possible causes of bad breath and does it mean there is serious underlying problem make sure you watch to the end the basic mechanism of bad breath is decay of food debris there is food debris on the tongue and in the crevices between the gum and teeth the decay of food debris gives the foul smell it is like leaked food in rubbish bin which decays giving out unpleasant smell the chemical responsible for the smell is called volatile sulphur compound this is a collection of a few chemicals which contain the element sulphur it is like sulphur deposit around hot spring you can recall the unpleasant smell of sulphur volatile sulphur compound in bad breath is similar with a characteristic smell this is the cause of bad breath it is present in the air we breath out and detected by people around us there are some germs living on the tongue and the crevices around the teeth these germs love to live in these environment the little spaces between the papillae on the tongue provide an ideal habitat for these germs when you eat food is swallowed and food debris is left on the tongue or around the teeth especially if you eat high protein food protein is made up of amino acids and some amino acids contain sulphur when the protein is broken down sulphur containing amino acids are left behind maybe in the crevices around the teeth under the action of the germs there volatile sulphur compounds are generated causing bad breath you may notice after drinking protein rich milk or soy milk not long after you will notice that there is a foul smell in the mouth this is because of the action of germs on sulphur containing amino acids forming volatile sulphur compounds other than on the tongue back in the throat there are 2 little structures on the sides the tonsils; they sit in a deep pit on both sides at the back of the throats there is quite a bit of dead space where food debris and germs and hide if the tonsils are inflamed there will be bad breath even if there is no inflammation there may be stones in the tonsils yes stones can be formed in the tonsils it is formed by combination of calcium and food debris undergoing reaction by germs on in the mouth and the result is stone or tonsilloliths if there are stones you can see some whitish / beige spots on the tonsils sometimes they can even be coughed out if you really try and when you look closely it looks a bit yellowish a little bit firm but is brittle you can break it by applying some pressure when you do this and smell it it is terrible these are tonsilloliths if there are many this can be a cause of bad breath we can safely say that over 80% of bad breath is caused by oral ie tongue and dental problems where germs act on food debris to give out volatile sulphur compounds causing bad breath now we know what is causing bad breath let's talk about how to tackle this problem oral hygiene is always first after eating make sure you rinse the mouth pay attention to dental health see your dentist regularly pay attention to peritontitis ie inflammation of the gum around the teeth as this is the main cause of formation of deep pocket around teeth germs and food debris may be trapped inside as a result volatile sulphur compound is formed mouth rinse may be used to kill off the germs here is an example 2% chlorhexidine mouth rinse is an effective way to reduce the number of germs in the mouth besides brushing your teeth make sure you always brush the tongue you may consider this little tool which can scrape the tongue just put it deep into the mouth apply a bit of pressure and gently but firmly pull it out like this to get rid of food debris on the surface of the tongue by limiting the number of germs production of volatile sulphur compounds may be reduced this is especially important in the elderly in this population tongue coating is a very common problem so advice your elderly relative of the importance of this other than oral problem there are other conditions which can also cause bad breath for example the respiratory system people with infection of the sinuses those little spaces within the skull pus collection may give out foul smell sometimes rare lung diseases like lung abscess in some lung cancer dead tissues within the tumor can give out a very unpleasant smell but these are uncommon there is a myth that bad breath can arise from problems with the digestive system like problems in the food tube the stomach or the bowel eg constipation actually this is not true; problems in these parts rarely cause bad breath besides some medical condition like diabetes especially type I diabetes where onset is usually in childhood in which there is NO insulin as you may have known in this video I made earlier if there is no insulin glucose in the blood cannot be utilized and the body is forced to use fat as the energy source this is not a very effective mechanism a lot of toxic substances when these are accumulated and cannot be removed by normal mechanism some of those wastes are eliminated in the lungs through evaporation in the air one of these is acetone in fact patients with type 1 diabetes the lack of insulin will cause a dangerous accumulation of toxins called diabetic ketoacidosis under this condition toxic wastes build up too fast and have to get out some way if you are really close to these patients you may be able to smell acetone, one of the toxic wastes and it is like the smell of nail polish remover notice that diabetic ketoacidosis is not commonly seen in adult onset type 2 diabetes another condition where bad breath is seen is serious liver diseases like liver failure where liver function has diminished to the end stage the liver helps to remove toxic substances when the liver fails and toxin substances accumulate some of these may be eliminated through evaporation through the lung causing a very foul smelling breath and whenever you see this symptoms this is a sign of imminent death that's all for today thanks for watching my new book Dr. Rex Medical Kindergarten is being reprinted the 3rd time if you have not bought it make sure you do when the 3rd edition is out please subscribe to this channel and press the bell leave your comments I'll see you soon
