let's talk about gout it is associated with high protein diet in middle age men who do little exercise protein in meat will be converted to purine in our bodies which in turn become urate; urate in high concentration will become crystals ie turn from liquid to solid in the joint fluid of a gout patient we may see needle shape crystals under the microscope the lower temperature in the extremities favour formation of crystals so gout tends to affect joints in the feet, hands or even ears these crystals will elicit an inflammatory reactions with pain, redness and swelling so a typical gout attack will be an acute red swollen painful big toe to prevent an attack avoid high protein diet shrimps/crabs/meat and offal are frequent trigger of gout moreover alcohol will raise urate level exercise more and ideal weight would be beneficial in an acute attack colchicine is the classic drug it is very effective but vomiting and diarrhea are common side effects other than colchicine ibuprofen, diclofenac and acroxia are also effective treatment which can control pain readily a third option is oral steroids; this is because steroids are like ibuprofen/diclofenac they can contain an inflammatory reaction a few days of steroids will control most gout attacks but this is not first line after an acute attack one may check blood to confirm the diagnosis blood test is less accurate during an acute attack it is best to check blood a few weeks later once gout is confirmed you may consider urate lowering drugs a classic example is allopurinol which should be taken long term it is safe and effective but occasionally may cause serious skin reactions another option is the new drug febuxostat which is much stronger but it is not suitable for people who have had stroke or heart disease notice that when you initially use these urate lowering drugs you may have a gout attack better have some antiinflammatory drugs with you in case this happens thanks for watching; please like, commend and subscribe.
