so this is residency it's mostly this hello I'm off to go to ride a foot Yeehaw with a little bit of this if time allows lots and lots of coffee like all of the coffee in the world sleeping when there's time and a healthy dose of anxiety [Music] already first clinical day at residency oh boy hello for those of you new here hi my name is Kian I am a first year vascular surgery resident okay I'm heading home I think this means I officially survived my first day overall I would say today was a good day well it's three in the morning I have not gotten a lick of sleep first official day on call has not disappointed just about to close my eyes and hopefully get some sleep I just like I'm having trouble wrapping my head around the fact that this is going to be my career and it feels amazing [Music] good morning so just walking to the hospital I'm on call this weekend life is good it's still light out foreign so I just got to do my first or case with my brand new Loops I feel like this is a rite of passage as a surgical trainee the box is like kind of ridiculous I feel like it looks like a lunch bag it's like so big but let me show you the loops they essentially magnify your operating field so you can see things and it doesn't sound that impressive but actually like being able to see the tissue details and seeing things that large is it's so cool but it does take a bit of calibration because your hands look huge and yeah it's just weird that's what I look like with my loops on and then there's a headlight attachment so that I always have light but yeah it feels pretty fancy and then the reason why the box is so big is because there's also like battery packs for my light this is the view through my Loops so you can kind of see like all the details of the fabric and then if I take this away that's like how far away stuff is in real life hello it's another night on call 2 30 a.m it's been busy we got to do some operative things and yeah I think one of the best parts about vascular surgery is you get to like help out your colleagues a lot when they get into trouble with like major bleeding and I think that's like pretty rewarding so I'm home in my natural habitat which is literally on the couch it was such a lazy blob I just finished probably the longest surgery I've ever been in uh in my career so far by the end I was just like suturing and I was like I don't even know what's happening anymore this was a multi-disciplinary case it was a bunch of vascular people I think at one point there were four vascular surgeons including like myself and other residents and then to people from hepatobiliary but to be honest with you I freaking love surgery and I had so much fun it was Saturday and there's honestly no other way I'd rather spend my Saturday night I'm walking home gonna treat myself to some post call Egg Club I love their sandwiches they're so good I'm riding on that you know 24 hours no sleep actually has been way more than 24 hours it's like 30 hours enough sleep but adrenaline High Am cortisol is kicking in I feel alive but I know like the minute I finish eating and get home I'm gonna crash so hard it's gonna be coma Kian s [Music] good afternoon I guess and post call I've caught up on my sleep so I'm gonna try and have a semi-productive day [Music] foreign [Music] so when I'm on call I try to get outside or at least look outside of a window today it's uh the window hello I just finished work so I'm running to try and catch a workout class because I don't know you gotta exercise when you can okay I got two minutes almost there so I'm on call tonight and I'm heading to a different hospital to operate we have two operations going and two operative teams finish that candies I got to do so much I did my very first ephemeral artery patch and oh my gosh I was like this is insane I'm operating on somebody's artery this is like what vascular surgeons do alrighty we are back on Home Hospital territory it weirdly feels like home away from home which is super depressing to realize but here we are I'm gonna get some sleep [Music] so I'm coming to the end of my vascular chunk and I'm feeling very reflective it might be the lack of sleep but I don't know it's just interesting to look back on the past four months I had so many doubts when I entered residency I had doubts about my own abilities but also like pretty significant doubts about the overall decision to pursue vascular surgery I think a nagging thought that I had in the back of my head a lot at the start was do I actually love this enough to dedicate my life to this which is definitely something I thought I had figured out when I applied and and when I went through medical school but I I think it's very hard to know for sure until you're in it I guess there was a part of me that was war like once I was fully immersed in vascular surgery as a resident I I wouldn't like it but if you luckily I absolutely love it I was a little nervous about moving across the country you know leaving my friends family and support network but I feel a lot more settled here now and I have amazing co-residents and the staff that I've worked with so far have just been really amazing so I'm really happy with this decision and I'm really happy to be here
