hi everyone and welcome back to my channel for those of you who haven't been here before hi I'm Lori Hill and on this channel we talk about plastic surgery cosmetic procedures Beauty and beauty Wounds if you're easily offended by those images please skip this video I've long been open about my rhinoplasty Journey starting when I was 21 years old and I got a bad rhinoplasty from a plastic surgeon this led to subsequent revision rhinoplasties that finally led to my dream knows that Dr guyron was able to create for me it's so stupid to get emotional overnose but like all these years of wanting just to look normal like that's that's what I was waiting for like I was able to move on with my life you know I was able to stop thinking about my nose [Music] unfortunately the nose of my dreams was ruined last year in 2021 after I got necrosis to the tip of my nose and ended up losing it this video is for anyone who gets facial or body plastic surgery as well as people who get cosmetic dermal fillers to their face it's vitally important that you watch this whole video because within this video I'll tell you about my nose necrosis situation what I wish would have been done immediately after the vascular occlusion was discovered and Vascular occlusion is something that leads to tissue death necrosis of tissue I'll also tell you what I wish I would have known in order to possibly even prevent the necrosis from happening the treatments I used were too little too late to save my nose tip but if you listen to me and do what I wish I would have known to do you can potentially save a body part or a body part of your loved ones let's get started so this happened to me in April of 2021 and it really seems like a lifetime ago because I've been through so so much now I will be telling you everything about the situation but I will be leaving out the surgeon's name the reason I'll be doing this is because I firmly believe that this could have happened in any surgeon's hands it really had nothing to do with the expertise of my surgeon or anything that they could have done differently the surgeon that I used was highly regarded had tons of education and had really good reviews so I feel at peace that I did choose the right surgeon for the procedure that I was having done which I'll get to in a moment the second reason why I won't be disclosing the surgeon is because if I were to disclose the name of the surgeon and they came after me with a cease and desist letter I'd have to remove the whole video and if I have to do that then none of you will have the benefit of knowing what to do in the case of necrosis I've actually had to remove a previous video where I reacted to someone's is plastic surgery because their plastic surgeon went after them and then they went after me to remove my reaction to their plastic surgery so I definitely don't want to put this video at risk for that even though I don't think this surgeon would do that I just don't want to risk it so in April of 2021 I had a fat transfer performed to my nasolabial folds this area right here so once the surgeon had harvested the fat they then injected it into my face the procedure went without a hitch nothing was weird or off about the procedure and I definitely didn't feel anything extraordinary on my face I went back to the room and recovered with a nurse who was set to stay with me after the surgery I will say that the first line of defense should have been that nurse she should have noticed that I developed a reddish blue mark on the tip of my nose and that should have been caused for alarm for her but unfortunately she didn't notice anything out of the ordinary or at least not enough to comment about it and she stayed with me without really alerting me or the doctor to anything being off now I really had no idea that the reddish flu mark on my nose was the first sign of a vascular occlusion a vascular occlusion occurs when blood is no longer able to pass through a blood vessel it may be a complete occlusion or partial occlusion resulting in a diminished blood supply a vascular occlusion may be caused by an internal obstruction such as the blood clot of foreign body such as filler material or fat or occur as a result of external compression what will happen if it stays occluded is that tissue will die it's called necrosis today's nose and they did comment on my nose tip being bruised though they didn't seem overly concerned about it I do remember that necrosis was mentioned in an off kind of way sort of like a throwaway comment but again not overly concerned and I believe I pressed them about it and I said are you worried at all and they said ah very little percentage am I worried about that that's so rare and they sent me home and just let me know to update them on how my condition was so I flew home and when I got in my car at the airport I took this photo where I really noticed that there was a definite large bruise on my nose also a side note in addition to the fat transfer I had to my nasolabial folds I also had dermabrasion to my lip lift scar which is the red area you see here around the base of my nose this is not any kind of lip lift revision it is only treatment to an old existing lip lift Scar the next morning I definitely knew that something was off because I woke up looking like this and I was feeling pressure and a bit of pain even and after my surgeon saw this photo they said they weren't sure what was on my nose but they thought it was a bruise and although I probably didn't have an infection they would prescribe antibiotics just in case in addition to the steroids that they were prescribing they also recommended for the first time that I look for a hyperbaric chamber to get hyperbaric oxygen treatments but they didn't mention that the reason for these treatments was because of suspected necrosis to my nose now this was four days after the initial injury to my nose which is a significant time for the nose not to have the vital oxygenation it needs for its tissues I was able to start the hyperbaric chamber although it was a few days later and by that point the necrosis had really Advanced but I was able to stop the progression of the necrosis by starting my Hyperbaric sessions at least the necrosis didn't reach my whole nose and it had only really affected the tip of my nose tip number one is to find a hyperbaric chamber a hospital grade hyperbaric chamber now this might be difficult it was hard for me the problem them being is that there's a lot of places that advertise themselves as hyperbaric chambers but they're not medical grade places that have chambers that look like this for example this is not hospital grade or medical grade this is the type of chamber you want to look for this is a hospital grade hyperbaric chamber by the time I was able to get to an actual hospital that had a hyperbaric chamber it was a few days later and now getting approved for therapy was an even bigger deal than that because hyperbaric chambers are used generally for people affected by diabetic injuries they weren't used to seeing plastic surgery patients not that it should matter I really don't think it should matter how you got that injury whether it was plastic surgery or diabetes yet there is a stigma there but thankfully the doctor that I saw understood that I needed help immediately and she scheduled me for Hyperbaric sessions now it's within this first week of dealing with the necrosis that one of my plastic surgery professional friends suggested that I try to get nitrogel Nitro gel is nitroglycerin gel I think it's generally used for heart patients and when it's applied to the skin it opens up the blood vessels so that blood blood can flow to the tissues this is tip number two if you think you may be having vascular occlusion leading to necrosis get some nitrogel and so I did ask my surgeon for a prescription for Nitro gel now I did try Nitro gel but unfortunately I don't think it did much for my already necrotic nose as you can see it had progressed by the time I tried the Nitro gel now had I applied this the evening of surgery and gotten that oxygenated blood to the tip of my nose I really think this whole situation wouldn't have happened I think this gel is maybe around ten dollars a ten dollar gel could have saved me thousands upon thousands of dollars and a whole year of Heartache you should have this on hand just in case now going back to the actual procedure I had the reason for the vascular occlusion or the blocking of blood to my nose was thought to be due to a bit of fat breaking off from the fat transfer that was done and then going into my vessels and then clogging up a vessel that brought oxygenated blood supply to my nose if you take a look at this depiction of the vessels of the human face it was likely that the fat got caught in a vessel that traveled and blocked a facial artery that supplies blood to my nose now this is very similar to what happens to BBL patients who get that fat transfer to their butt and a bit of fat breaks off and travels to the lungs or the heart and can cause death so now you see that although it sounds very innocuous fat transfer could be really really dangerous and I really learned that last year when this happened now you should also know that getting filler to your nose or filler anywhere in that area also counts it's not only fat that can include a vessel filler can also occlude a vessel although they could put in something that reverses the filler but there's still problems with that sometimes it doesn't all dissolve and there are many filler accidents reported online take a look at some of these photos of necrosis just from filler I would caution you very severely in unless you have a very experienced injector doing filler to your nose I know for example Dr rivkin talks about the way he does filler to the nose and he says that he's constantly moving so he's not in one place too long when he puts in the filler but very few practitioners can do what he does so unless you're seeing Dr rivkin or somebody very similar I would really caution you not to have filler to your nose and to just get the rhinoplasty instead additionally I would really caution you not to have filler to your face especially your nasolabial folds and remember filler can mean dermal filler or fat take a look at this example this patient had dermal filler to her nasolabial folds they had hyaluronic dermal filler now directly after the patient complained of severe pain they immediately gave the patient reversal medications to reverse first the hyaluronic acid and other medications including nitrogel and they started them on hyperbaric oxygen unfortunately the necrosis still progressed and did a lot of damage as you can see now at the end of 43 Hyperbaric sessions and all the advanced treatment they could give her she still ended up losing half of her nose and keep in mind her nose was never injected with the filler to begin with the damage was thought to have occurred when the right facial artery was occluded or blocked with filler now back to the Hyperbaric sessions I went three times a week for many months and I basically structured my whole life around these Hyperbaric sessions thankfully we were still wearing masks at the time so not that many people saw my face as it was now my nose ended up developing a hard crust on it a hard Dark Crust and I didn't know what was under it and I knew I had to wait for that crust to fall off to know what was there it could have easily been a hole I didn't know what was there now while I was going to these Hyperbaric sessions I was also going on consults to reconstructive doctors who all told me that I would need a forehead lap that looked like this during a forehead flap surgeons lay a flap from the forehead and use the forehead's vascular blood supply to feed the tip of the nose to achieve new circulation for the nose and yes you must walk around with the flap attached like this for a couple weeks this is a three to four week process and can be very expensive in addition to leaving an additional forehead scar now this procedure is the gold standard for nose reconstruction and has come a long way but even so I really hoped that I wouldn't need to have it one Secretary of the doctor even went as far to say as she didn't think that I would be able to continue my career on camera after it I was devastated you guys like I don't usually cry in the face of challenging situations but I definitely cried I wept because this is so important to me my career is so important to me that I just couldn't fathom never going back to it it was such an awful uncertain time but I was told multiple times that although I would likely need this forehead flap reconstruction where they cut from the forehead and they take some tissue and then attach it to the nose and give the nose the blood supply and they then they cut it off even though most doctors agreed I would need this they all said I had to wait until the hard crust that developed on my nose fell off to see what was underneath so I knew there was a bit of hope so I did everything possible I made every Hyperbaric session I ate healthily I did everything possible I prayed I just did it all I just knew that if there was this one chance it was that and then finally the day came where that hard crust fell off and I was so scared to see what was underneath and then when I finally looked I saw there was pink skin under that scab and I was just so full of joy I was so happy because I knew that meant that I would likely not need that forehead flap reconstruction [Music] something but when I turned to the side I did see that the tip of my nose was completely gone and now I had this blunted tip to my nose but I knew that even with that blunted tip I could still go on camera now after I finally did go on camera many people in my audience assumed I had gotten another nose job and I got many many mean comments saying that what am I doing to my nose and I need to stop messing with my face little did they know that I didn't do anything to my nose and that I had suffered a horrible injury and this whole situation really affected my self-esteem deeply I was scared to ever show my face I wore that mask even when I didn't need to wear it it just really shattered me it was really a hard hard year now as a side note my surgeon did pay for all my Hyperbaric sessions and for everything I needed afterwards and they even paid for the later repairs that Dr guyron made to my nose so tip number three if you've had any sort of rhinoplasty's done or even a lip lift be very cautious of doing any further work to this area of your face as the blood supply may be already compromised for those of you who have had more than one rhinoplasty be very careful about any kind of procedures that you get near your nose because your blood supply could already be compromised from past procedures if you've had a lip lift be very careful about having any sort of subsequent work in that area because even a lip lift compromises the blood supply to the nose because because of where they cut right here so be very mindful if you're a past rhinoplasty or Lip lip patient getting subsequent procedures to that area of your face let's talk about something happier the revision I got on my nose necrosis so I went to see Dr guyron Who was the surgeon who gave me my dream nose in the first place and he was extremely empathetic he really listened to me he told me not to be too concerned about what's happened in the past because we can't change the past and he's right and he examined my nose and told me that we will indeed do something but we're going to do it in steps again it is unfortunate that this has happened there are things that we can do I'm going to do as much as I can in the first operation but we may need to have subsequent procedures just to refine it a little bit more or get more uh projections slightly more length make it look more natural and fortunately there's not a major major surgery is not a lengthy operation but we just have to be a clear about it that we're taking a little bit of chance we may require some hyper back oxygen after the treatment after the surgery and I'm going to take a piece of cartilage room behind this projection it's called the tragus because we have had surgery in the years before and unfortunately I don't have any Carthage that would be suitable for what we're going to be doing yeah any questions yeah what do you think the risk of secondary necrosis would be in the surgery yeah I'm going to do as much as I can and within my power my experience to minimize that chance um and I don't think that it's really hard but we are taking a little bit of a chance but if we see that there is any potential for not having enough philosophy to the skin you may have to again go through the hardware treatment with hydrobatic oxygen which will enhance the circulation and reduce the potential for loss of teaching I'm not expecting a lot of surrounding the minimal swelling and you would have seen ointment or I mean it didn't use any bandages on this because we want to watch closer or if I use bandages basically we will have some parts of the tip to be exposed so we can monitor the circulation of the skin you know that I'm going to do my best yes you always have yeah you save my nose once you can save it again um so I had step one surgery this last summer where he took tissue from my tragus area which is right here on both sides and he put it in my nose he did what's called undermining of my nose where he didn't cut here he only put tissue in here and here and he propped up my nose to have more projection he wanted to see how my nose would handle that and then we would move forward with future surgeries now I gotta say I was so worried about what would happen but after surgery once we knew we were in the clear and that my nose had handled this new tissue well I just felt this Rush of relief take a look at my results from the surgery with him [Music] foreign [Music] as I was about these results he did let me know that I would lose some of that projection because it's just normal with the tissue he put in but that with this surgery being successful we knew that we could proceed with a step two surgery and I will be seeing him in a few months to discuss that but I am so happy with how my nose looks now and part of me is even considering not doing anything more to it just because I don't want to risk anything else so I want to go over the three tips again so that you can prevent necrosis number one hyperbaric chamber if you suspect vascular occlusion it will not hurt to go to a hyperbaric chamber to open up those vessels and get the oxygenated blood to the tissues number two if you suspect you have vascular occlusion after seeing your practitioner and having them look at it ask for a nitrogel prescription number three don't have multiple procedures that compromise blood flow to your nose or around your nose I just down below for 20 off thank you guys so much I love you all bye in general how long is recovery from revision rhinoplasty no it depends on the magnitude of the surgery it could be a week of social recovery it could be 10 days and the way it works and concerning my experience with this operation I don't let the patients to get a lot of bruising swelling around here is because they use medications during the surgery if I feel that the patient is going to be bruising a lot but if I have to use the for example the rip cartilage there is some discomfort from the real cartilage removal and last couple of days if not to two weeks they may have some tenderness in the area and they can't participate in heavy sports playing tennis jogging swimming golf for that period of time and we asked the patients that you know not to take any aspirin aspirin dark medications for all of our cases surgeries that would apply to the religious surgery as well uh the bandages to stay on usually seven eight days uh and many of the patients may have tubes inside their nose that will be removed during that seven eight days all of my stitches are dissolvable so there are no stitches to be removed we still ask the patient not to take aspirin aspirin type medications again in the strenuous exercises they have to refrain from Beyond that actually they can walk around and they react we encourage the patients to walk around even the night of surgery the next day as much as it is within their comfort zone and the more active they are in that regard but not against radius activated walking and light exercises over there is okay as long as there's a desk disk job and it is not a strenuous work that it had after the bandages come off most of my patients can go back to work and today with the covet and virtual working from home many of these patients can actually recover with the mask on they can cover their nose that's true and get back to work a lot faster and last question when can somebody start their skin care routine again um that is okay seven eight days after the surgery but they won't be able to wear glasses for about five weeks without using some mechanism of support of the nose we don't want any uh direct contact between the glasses on the nose while they are recovering the nose is usually numb for a long time as you recall from the previous surgeries and that is something the patients have to be prepared for taxi somewhere around three to six months for the anonymous of the nose to go away okay great okay well thank you so much again
