good evening welcome friends family faculty staff students and particularly the penn dental medicine class of 2024 we are gathering here today to celebrate the white coat ceremony for the class of 2024. actually because of these strange times we're not gathering at all we're socially distancing as much as i wish we could welcome all of you here to pendental medicine for what has traditionally been a warm welcome to the new class their family and friends this year due to the pandemic were unable to do so i am standing in the evans building of the school of dental medicine here at university of pennsylvania it is a historic building at a historic school made possible by the donation of thomas evans upon his death to establish a school second to none penn dental medicine has become just that a school second to none when this building first opened in 1915 it was but a few years before the spanish flu pandemic that killed millions worldwide just three years after the building opened the pandemic hit the united states and in september of 1918 it spread rapidly through philadelphia after a liberty bond parade which initiated outbreaks throughout the city and killed tens of thousands it was a time before science really understood what the flu was and how the flu spread and even more what we could do to treat cloth masks were used by health care professionals to protect themselves not even as much hand washing and cleaning the population was instructed not to spit as it was thought that the spit was the major transmission vector physicians recommended the well-known remedy of cucumber sandwiches and whiskey concoctions to help treat the outcomes of the disease it was unfounded by science and and even more was just anecdotal with today's pandemic we know that social distancing wearing a mask hand sanitizing as well as avoiding large group gatherings are all as well established methods for preventing the spread of this life-threatening disease this white coat ceremony it's special the white coat it does something to you um it's the only white that you're actually able to wear after labor day um i think i look thinner in it maybe a little bit taller but definitely people see me in this and think i'm smarter maybe the white coat is like the secret handshake to a fraternity or sorority welcoming us into a new club i'm positive it will help me get a reservation in a restaurant particularly the difficult restaurants i think the white coat ceremony is a really special event my name is mark wolfe i am the 12th dean of the school of dental medicine at university of pennsylvania the white coat ceremony being performed during the first days of your professional education before you really understand what it means to be a doctor is kind of strange but today during the pandemic it is more important than ever the white coat ceremony is actually a ritual where the new student accepts the responsibility of being a professional and i tell you now more than ever it's a 24 hour 365 day a year responsibility health providers did not always wear white coats through the late 1800s actually physicians wore black coats even strange face masks with long noses to ward off evil spirits it was in the late 1800s that after after somebody like joseph lister discovered infection and and germ theory that doctors started wearing white coats as a a sign of cleanliness it was also as they wanted to be assumed to be a consumer of science and academia white coat ceremonies actually taking an oath of professionalism became part of health education in the 1990s 100 years later so as it turns out the white coat ceremony is actually a ritual it's like placing a wedding ring on a finger and it symbolizes a commitment to somebody to something but what does it symbolize people are going to know that you're important but wait i believe the meaning behind the white coat isn't about what it does for you it's about what it means to the people who see you and and understand that you're wearing it or even when you're not wearing the white coat you are a professional wearing a white coat says more to a patient than i follow inspection control guidelines as lister intended as a matter of fact most patients do not associate the coat with infection control they associate the white coat with something much much more wearing the white coat does say to the public that you are smart and well educated we know you are smart and university of pennsylvania school of dental medicine is going to help you be the best educated practitioner in the world wearing the white coat says to the public that you will spend your lifetime lost my place sorry that you will spend your lifetime [Music] where spend your lifetime learning and staying current that you will understand the reason why you do something not just temporarily learning the answer for a test your patients know because you wear the white coat that you will always use the best evidence to treat them wearing a white coat says to the public that even when you're not wearing the coat you are a doctor you're always a doctor when you're sitting on an airplane and there's an announcement asking for a doctor to help somebody who's ill or a tornado strikes your town and emergency care is needed by others you are expected to be the one to step up the white coat tells the public that you are always a doctor and will be there to help them whenever it means that you do what is necessary to protect them and never do harm even when you're not at school not in the office and not practicing and today it means you sacrifice some of the things we normally do to be social we need to wear the mask socially space and avoid gatherings like giant white coat ceremonies wearing a white coat says to the public that you care about your community the healthcare system for all even the underserved the community you live in as well as the communities around us as well as our own profession we need to care wearing a white coat says to the public that you will always place your patient's best interest ahead of your personal interests and you'll practice with honesty and integrity in all aspects of providing care wearing a white coat says to the public that you provide compassionate treatment for your patients respect for their dignity and privacy no matter who your patient may be truly wearing this white coat means a lot the oath you take today comes with some awesome expectations traditionally your faculty place the white coat on you your faculty believe in fact they expect that you will fulfill the public expectations of who you should be as a professional and wear the white coat participating in this ceremony and putting on the white coat means that you agree to fulfill the mandates of our profession putting on the white coat is much heavier than the fabric it represents everything we expect our pen graduate to be is part of this white coat congratulations to you and your family and friends this is your first step into the profession oh yes one more thing about the white coat it won't help you get the reservation or a better grade for the examination my recommendation is you better study it's at this moment that i get to do something that i take incredible pride in doing i would like to introduce our guest speaker for the white coat ceremony dr fred ferguson he's somebody very special to me fred ferguson is an assistant director of the pediatric dentistry program at interfaith medical center in brooklyn and he's a distinguished teaching professor emeritus at the school of dental medicine at stony brook university he's also an associate professor of pediatric medicine at stony brook during his 42 year career at stony brook dr ferguson developed an oral health program for special needs patients including children with hiv infection chronic health problems and craniofacial deformities while also maintaining an extra and an extra mural practice he taught that hundreds of dental students dr ferguson is a highly sought after speaker who regularly presented to the professional consumer and community groups and has consulted on healthcare topics with hospitals organizations and corporations dr ferguson is president and ceo of and as well as founder of health migration consultant consulting and about smiles world limited these organizations are focused on improving the quality of care and reducing the cost through building a process for consumer accountability stakeholder collaboration and the electronic medical record integration fred is more than that to me though fred was my professor when i entered dental school and throughout dental school and residency he taught me pediatric dentistry and he taught me a lot more than that the most important thing to know about fred is never play any sports with fred he sent more students off basketball courts with injuries secondary to elbows tough game ankles things like that held a special place in my heart when i broke my elbow as a student in a student faculty softball game three weeks before graduation um sliding into third base fred comes over looks at it and says maybe with a little pressure we can pull it back in place and we can get back to the game just in case you wanted to know i took my boards in a full arm cast fred taught me that addressing him by his first name fred was not disrespectful but rather demonstrated my progression to being a professional fred taught a generation of dental students that treating all patients patients that that were allowed patients with disabilities was a professional obligation of all of us as the disabled community is our most neglected and underserved community he was amongst the first dentists to step up to treat individuals with a new frightening communicable disease that we knew nothing about for many years after it first came about today we call it hiv aids fred helped teach me the value of being a dental educator and changing a generation of practitioners in no small part my commitment to the people who are persons with disabilities is as a result of fred and i hope that i can impart this upon generations of students to come i would like to introduce you to my friend and colleague dr fred ferguson good afternoon and welcome and thank you dean wolf for the opportunity to participate in this celebration and to the class of 2024 congratulations on your achievement and to your families your partners and friends we sent you this image and requested that you spend some time reviewing it and to address the question what is the problem further we also ask that you consider that some of the individuals in this line might have family members with special needs or work at a community based organization that serves its population we could easily spend an hour dissecting what's going on here my opinion is that this image presents the the problems confronting our health care essentially we are engaged in our health care as patients that is we present for care when it's a problem rather than working with our health care over the lifespan confirming that we are in support of our health our healthcare essentially operates on illness rather than prevention and thus vulnerable communities and at-risk individuals predictably suffer chronic disease a significant obstacle also is present for oral health and dentistry and that is it's perceived low value in health care i would like to offer a solution in the year 2000 a physician dr shyam had published a paper the common risk factor approach a rational means of prolonging oral health and he presented that essentially that we are focused on the center column of concerns that is common chronic and costly diseases which are predominantly driven by the variables surrounding it in the beige globes but i want you to note that the all of these issues are directly or indirectly involved with the mouth above there is a the title of the foil is oral systemic connections i have some homework for all of the entering students and anybody that's on this conference and it is i would like you to go to the external hard drive google and read about oral systemic connections more time you spend with this you will have a real advantage when you start to encounter patients in developing that idea of a partnership essentially note that all of these endpoint chronic diseases in the center column are connected to all of these variables of the mouth going further in 2007 another physician a pediatrician dr fisher owens presented a image in which she organized these issues that we now understand on the atmosphere that we live in that both has heat and humidity and the more of these variables that you have in your life the hotter and more human it is and this is essentially why we continuously see the same demographics predictably suffering an unfair share of chronic illness but note that dr fisher owens has oral health in the center of this focus in their three levels child family and community so the second assignment for you to to focus in on is the phrase social determinants of health but i want you to understand that as tooth decay and poor health is the most common transmitted chronic disease of childhood as we get into teens and adults we see these other issues come in that we spend a lot of time reacting to predictably so we're saying there's this again there's this connection between your mouth and your body the third point i want to drive home is as i as i alluded to in the previous slide note that tooth decay and gingivitis are of childhood and through these behaviors habits and lifestyles which evolve become more numerous more complex more interrelated we then begin to see oral issues of the mouth that are more of a concern generate inflammation that we will not get into that's another subject entirely but note that these other issues that we see of chronic disease don't happen into your adults and this is a progression of these behavior habits and lifestyles so i propose that consumers and dental providers can play a key role in when i was in practice i had a lot of mantras that i presented to my patients and this was my favorite not just teeth like trees and gums don't lie if i know your age and your gender after puberty i can ask you some questions that i didn't make up they are well known in science and based upon the answers these questions i have a pretty good idea about what your mouth's going to look like so consider we have two sets of data we have data presented by consumers okay and then the dentist can answer some questions that they don't answer now and those two data sets are very closely associated so we now have means of getting early warnings of good health promotion by people or pure health promotion by people and i contend to my patients the most important health provider you have in your life is your dentist because a dentist knows exactly how you're doing every day in promoting your own health or causing problems so in closing i present that oral health can have a leadership role in healthcare with the benefits that you see here we go to being anticipatory proactive and preventive versus our reactionary health system which is not working the payers are having a real hard time with the issues of cost and value and this these issues are going to come to dentistry the most important thing is advancing population health covid showed us very clear that we weren't prepared our health care system can't work given the way it's it's it's developed it is reactionary and in any relationship the longer you wait to act the worse it becomes and what we really need to do is to recognize that for our most vulnerable communities and individuals at risk that we have to address these issues of disparities inequities and most important injustice so i propose that this information from susan from consumers and dentists working together as partners will advance health and advance dentistry's value i started a program in special needs care at stony brook university in the late 70s we've had a lot of graduates in that program and when i see them at meetings or just by just by chance often they will say to me fred when i included special needs in my practice interestingly enough their parents the people who brought them to me or if they worked at a community-based health center they would often return to my practice as a patient because they felt i am providing a valuable valuable care service through the personal special needs so i must be good again dean wolfe i appreciate the opportunity to participate in uh in this celebration and to the class of 2024. i wish you health success moving my warmest welcome to you in this important occasion i along with the entire faculty and the staff at pdm are really excited to have you on board all of you had the option to attend other dental schools and yet you picked ours you made the right decision in coming here and all of us will spur no efforts to validate your choice even in these unprecedented times our school has met the challenges imposed by the pandemic in an exemplary fashion our teaching clinical care and research experience are thriving and new initiatives such expansion of community outreach and clinic for here at penn dental medicine and i was so excited to welcome you to our school many months ago and now i'm even more excited to formally welcome you into our wonderful profession through the recitation of this professional pledge so now please join me in reciting the pen dental medicine professional pledge as i enter the dental medicine profession i vow to keep this pledge and these stipulations i understand that my primary responsibility is to my patients to the best of my ability i will embrace the highest standards of oral health care and treat all individuals under my care with respect and honesty their total health and well-being shall be my primary concern i accept the responsibility to maintain my competence through the continued pursuit of new knowledge and skills
