welcome back to another Vlog today's Vlog is going to be a little bit different at the moment I'm on my OBS andini placement I've done five weeks now so I've got three weeks left at the start of this placement they did say it's a good idea to have a go at doing a night shift I don't think everyone on my placement is actually going to do a night shift because we don't technically have to do one and you're probably thinking why would you bother doing that why would you do that to yourself but I thought you know what it might be fun it might be interesting so I actually spent a morning with one of the registrars and we got on quite well and she was really nice and she just said I'm doing nights on these days you welcome to come along with me for one of those she could at least make me feel involved the worst thing would be to go to this night shift and be The Annoying medical student that gets ignored so at least hopefully this way I'm going to have somebody who's kind of looking out for me so I have done a couple of day shifts on the labor Ward and I have managed to see a couple of births now including a C-section and and a vaginal birth with forceps so I haven't actually seen an uncomplicated vaginal birth yet so I would quite like to see one of those um because the forceps birth was a little bit a little bit stressful and of course the C-section was uh it was stressful as well cuz that was an emergency C-section I'm hoping to see some stuff tonight but we will see I'm going to take you with me it's also a little bit of a research thing for you guys so you can see what it's like to be a medical student on a night shift I think it's also important to let you know that I don't have to stay or night night I can leave probably when I want so I'm not expecting to be there till the break of the morning the time now is 6:00 so I'm going to make some dinner for me and Jack did I mention it's a Friday night I really really not planned this too well but the way I thought about it was it's one Friday night and I'll still get my weekend to recover ready for Monday so let's see how this goes come with me on my first ever experience of a night shift as a medical student [Music] dinner is ready you excited to eat that we we and we're going to watch I'll watch an episode of one day let me press play I am packing my bag I'm not bringing any laptop with me or anything because I don't imagine myself doing any work I'm not exactly going to go to the library amwh in the middle of the night I'm just taking scrubs name badge um plenty of water and I'm going to stop off at the shop I think and get some snacks because I'm a bit worried that the cafe won't to be open and obviously very importantly bringing my handbook to get signed off because we are not doing a night shift and not getting a signature that would be a nightmare you might be wondering why I'm wearing a Warick University hoodie um it's actually my little sister's I think she left it in this flat when she came to visit right I'm all ready to go I'm bringing my scarf and I've got my umbrella cuz I don't know what the weather's like out there I should probably just look out the window but I'm just going to become prepared for all weathers let's go get the train [Music] you just SP off this again [Music] I am so sorry to interrupt my night shift Vlog but I wanted to jump in here and I've just arrived it took me ages to get into the common room and I had the code written down somewhere but it took me ages to find it anyway we're in yeah it's 8:00 now wasn't that fun getting public transport here on a Friday night it was a bit A bit chaotic on the bus Handover is at 58 so I think I'll go down to the ward like4 8 just to make sure that I'm not late first I should probably get changed into my [Music] scraps this is what I'm taking with me to the ward because I don't want to take too much stuff my squash diet Coke obviously going to have to have plenty of beverages to keep me going strawberry pencils I've actually already tucked into and then my log book where I can get my sign offs done don't think there's much else to add I'm going to go down to the ward now um don't know how this is going to work don't know if I'll see you in a few hours or I'll see you in like eight hours probably not but you never know I'll try and update you as and when but if not wish me luck and I'll see you soon a few moments later what is the time let me have a look it is 10 1 in the morning I have not seen any babies being born yet so we did a ward round and that took quite a long time actually and then I was following the regge around for a little while just going to see different women different patients and then there's lots of may like there's a few moms who've consented to me being there when the time comes uh but the time just hasn't come yet and I think there could potentially be a C-section um at some point tonight a couple of births but I think we're still like an hour or so away from at least the first one of those so I was kind of just sitting with the doctors for a little while but I don't know if anyone else who's a m student gets this when you're on placement you want to get involved in the conversation but they're just talking about like workplace gossip or like stuff that you just can't really get involved in and like I'm not going to butt into their conversation if I literally don't know what they're talking about so I do try to get involved in the conversation but sometimes it's just a bit difficult and you don't want to be like over familiar I guess morning Jill morning Tracy how are you I'm good you good you good you good you morning Carol maybe I'm overthinking it I've got a cup of tea and I'm going to have a little snack now and then go back down I have left my mobile number and earlier they gave me the bleep as well but yeah so far I'm feeling all right I'm a bit tired but I quite like the night shift I like the fact that they were really happy to see a medical student doing the night shift and they are trying really hard they're trying really hard to let me see a birth it's just right now it's not happening but I think if I stick it out I'm going to see something it's just about waiting and being patient now I think So the plan is just to have a little break now and then go back down to the ward and see what's happening then but everyone is so lovely and there's tea there's Donuts there's cookies and I think because I worked in Hospitality like obviously I don't do like night shifts but sometimes at the pub I wouldn't finish until this time anyway so I'm not exactly in the Realms of like something I've never done before I will try and update you again the next time I see you it could be the end of the night shift to be honest um but we'll see how everything everything pans out cheers to tea oh I've got my diet coke as well 2,000 years later oh my goodness right guys it is 5: to 7: in the morning and I'm going home I'm going to go home and sleep and then we can debrief about the night shift because I feel so tired right now I don't think I could actually put a proper sentence together to explain everything I've seen so I'm going to go home and I will speak to you hopefully after I've had at least a few hour sleep hello it is me I'm back I've had some sleep and I'm feeling a bit fresher than I was in the previous clip the time is now 4:00 the following day I didn't end up getting home until 9:00 a.m. because I got to the station and I'd missed the [Music] 7305 train and then the next train wasn't till after 8:00 a.m. I had to sit in Costa and have a sausage bat because I was starving and I thought if I get home now and haven't eaten I will not be able to sleep so I had a sausage bath and a decaf tea from Costa and then I get the train home in broad daylight everyone is getting on the train to London ready for their big day out in the city on Saturday and I I am going home from my night shift and I felt ill I felt dirty once I got to bed everything just melted away and I fell asleep and I have managed to sleep for like what six or seven hours right let's debrief let's debrief I ended up staying the whole night obviously and I think at the start of the video I didn't really think that would happen I think my goal was to at least see one birth because we have to see a certain amount on this placement and if I was going to do a night shift the hope would be I'd see at least one I've definitely learn a lot about the unpredictability of Labor and how long it can go on for I spoke to you around 1:00 a.m. and then I went back down to the ward and that's kind of when things started to get a bit more interesting as soon as I went back down they were like Lydia where have you been you need to get down to theater you're going to scrub into the C-section and so I went down to theater and I got to scrub in and assist with the second half of the C-section I was hoping to have been able to like assist with the whole thing but because the baby was in the breach position apparently the C-section is slightly trickier basically the sh was assisting the regist and then she left once the baby had been born and then I got to join in and do all of this bits afterwards so and that went well and that was fine there was no complications um it was a category 3 C-section so that's one that doesn't have to be done like urgently urgently but needs to be done kind of as soon as so that was around 3:00 a.m. I think that was all finished the regist was like Lydia you can go home now like you've seen you've seen one when I got back up to the world one of The Midwives said to me oh you should stick around I think maybe this lady's going to give birth pretty soon but we can't say for sure I hung around and and I had another donut to keep me going one thing I've learned is that sugar and sugary snacks maybe aren't the best snacks for night shifts because you get like a high and then you crash and then you have to have more and I think actually the best way to do it would be to eat nice healthy snacks and lots of water then I go and join this Midwife and she was amazing like just to watch her kind of help this woman through the birth was just like so incredible to watch Like The Midwives are really amazing and what they do the bir ended up being an instrumental birth and I mentioned at the start of the video I've already seen a forceps delivery which is one type of instrumental delivery this instrumental delivery was a vont so that's essentially where they put a suction cup like a vacuum suction cup on the baby's head and they have to pull it out it's less risky to the mother than forceps but it is more risky to the baby the vont because of the pressure that's going on their skull and obviously the bones of their skull aren't formed like properly yet so it is more risky to the baby and I believe you can only have three attempts at pulling the baby out and if that doesn't work then you move on to forceps or emergency csection usually an instrumental delivery happens when the baby is becoming distressed or the mother is becoming exhausted because she's pushing and the baby's not coming out the doctors have to get involved when it's an instrumental delivery as far as I'm aware and that's what happened here eventually they decided that it needed to be an instrumental delivery and they did the vont and it was a success and it was amazing to watch because just the way that there was like three midwives came into the room two doctors everyone just kind of like acted really quickly like they had to get the baby out quite quickly cuz it was becoming distressed just like watching all that play out and the fact that they were doing that at the end of their night shift as well so that was the second birth I got to see by the time that was all kind of over it was around 6:30 the consultant was like right you can go home now let me sign you off and no joke as she went to write her signature the emergency alarm sounded and everyone just started running to this room so I followed them to the room and they assess the situation and luckily it was not an emergency it wasn't anything too serious in the end that was again a really good learning experience to watch them deal with a potential emergency scenario and really quickly explain to the mother they might have to take her down to theater for a C-section so watching that again even though I was tired it was a good learning experience and I'm glad obviously that in this scenario nothing serious was happening and once they had assessed the situation and figured out that everything was fine um the consultant turned to me and went you can go home now for real that's an overview of my night shift I would totally recommend doing a night shift if you're a medical student I can't recall another day on placement where I've been given that much attention and felt that welcome which is just I guess quite refreshing because there's no other medical students about they you know appreciate that you've given up your night to come and see what they get up to at the end of the day I am going to have to do night shifts when I'm working as a doctor so it's good to just give it a go and kind of understand what that does to your body how it makes you feel and what you might have to kind of endure on a night shift if you're a medical student and you get the opportunity to do a night shift be that because your supervisor has said come with me or because they say on the placement you should definitely get one done then don't shy away from it I would definitely give it a go but remember that you're not being paid and you don't need to like yourself through multiple nights and if you do go on a night shift and nothing's happening like it's absolutely reasonable to leave in my scenario it just so happened that I ended up staying the whole night but it wasn't that bad in the end I hope you guys found this interesting and got some insight into what a night shift might be like don't forget to hit that like button if you
