that was stupid complacency channelized attention task saturation misprioritization overconfidence and attention to detail these are all human factors that can cause or contribute to mishaps especially some that we've talked about on this channel with accident investigation board report reviews some of those factors happen to me on this lesson and today it's going to be more of a debrief uh than the format we're used to because i don't know if you noticed in the opener but that was probably within seconds of being a fatal [Applause] mishap well so i want to go to eunice but the there's rain there's rain everywhere all around the airport all thunderstorms i don't know if i have a clear shot um i don't need a cross country so i may just fly around for a couple hours but uh not looking great this uh summer weather pattern is not helping us today so you may have noticed a recurring theme since i've soloed which is bad weather the last few weeks have just been a soggy mess in louisiana it's been thunderstorms typically in the summertime weather patterns we get thunderstorms in the late afternoon but lately it's been mid morning midday and on with these pop-up thunderstorms that build rather quickly you notice that on my solos my first solo and then uh the solo cross country i just did on this day my plan was to go out to for romeo 7. eunice louisiana was going to go meet doug for lunch to fulfill solo cross countries even though i had already done my solo cross country de facto by going to hammond i met all the requirements necessary i was going to go do another one because i needed to burn time and it would have been cool to see him unfortunately weather as i arrived at the airport was moving in all around and it was blocking the way to units and one of the things you have to realize about flying helicopters you can't really deviate like you can fix wing because fixed wing you can climb and have good glide ratios to other airports and stuff helicopters you really don't have that option and going to eunice there's really only one route because otherwise it puts you right over the chaffala basin a lot of water a lot of swamp places you don't want to have to auto rotate and land if the engine should quit so you got to have what lester calls the oh spots and there's really only one route to do that safely while maintaining some semblance of a reasonable time there and back so it was blocked and i could not make it so i pulled the helicopter out pre-flighted it was ready to go but the weather just wasn't cooperating lester showed up he was actually going to the airport to go fly with someone else and the rain showers arrived so i couldn't even fly anywhere so he and i went to lunch at the nearby cafe in the terminal which is a great place real real good and then once that happened the weather finally cleared out but it was too late for me to actually go cross country and it was still blocked by storms and stuff to the west so i decided i was just going to fly around locally as i had done yesterday and burn off hours and just fly my new plan was to do the river run to ema which is a little grass strip where i used to have an airplane we went there back in a previous vlog and potentially fly some more just to burn time just to enjoy the sights of the city and then come back the corn airport information tango 16530 1508 visibility one zero thunderstorms in the vicinity few clouds of five thousand six thousand five hundreds gap [Music] [Laughter] [Music] just want to make sure thunderstorms in the area but the field is currently uh it's not raining or anything current winds are one windsor at niner thank you hey standing one warning like front tower helicopter five three six sierra hotel from a signature would like to enter a left closed for the triangle we've got think of [Music] make a less close traffic to the southeast triangle traffic to the southeast triangle on a risk of six zero two all right we're right or left here we go 3ds topped you know wc i hook up to six hotels south east triangle cleared for your option just remain east of taxiway bravo at all times east of taxiway bravo the south eastern cleared the option six hotel bird you can continue inbound for now at traffic making the left close to the east side of your helicopter biggest final one searching for that traffic sir your hotel traffic just about a mile east of the field in benfica fox where the beam signature is a helicopter 500 feet secure tail has a traffic inside we'll uh delay that one with 27.18 right now wait flank one eight right one more [Music] seven point seven i want to do a couple patterns and then i was going to have lakefront tower coordinate a squawk and clarence and bravo so that i could fly riverrun oh boy to the west how do i get thrown around by that six-year-old tells that request left closed back to the track hey medevac one with your top flight we're heading runway one eight right clear for takeoff well we're heading right let's traffic include the option for the option southeast triangle i'm completion i'd like to do a river run at this time i've coordinated with lakefront to get a squawk and to get into the bravo airspace and as i'm coming around they start talking to me you can now see the swiss cheese model starting to line up that's another steve so i realized it wasn't going to be the best normal pattern as i'd planned and i decided well i'll just do a steep approach they asked me if i was going to be at 1500 feet i said yes i'm still focused on this approach now i'm getting inside of 200 feet approaching the land it's a steep approach i'm working my parameters trying to get it right remember steep approach highest probability of a settled with power or vortex ring state and at this point tower gives me instruction hey helicopter six here tell us you can squawk and departure frequency will be 123.85 so they give me a squawk code and a frequency at this point many human factors come into play one channelize the tension i'm listening to what they're saying task saturation i'm trying to fly the helicopter but also trying to add the other tasks that they're giving me misprioritization now instead of flying the approach i decide to take my hand off the collective and punch in the squawk code overconfidence i believe that i could do it i thought i had the approach wired and i could do i could take my hand off for a quick second this is another fixed wing crossover because in fingertip you're expected to make frequency changes uh and in the t38 you know it's a manual frequency change so i can be in fingertip take my hands off do that and then get right back to flying so it's one of those things where subconsciously you get a little too confident in your ability in your own abilities and i had been progressing in helicopters such that i was like well i think i just felt like i could do it and i felt like i didn't need to say standby so in doing so i leaned forward which then caused me to go half cyclic already at a slow state and my hand was off the collective and now i get into a saddle it was not a full settle with power or vortex ring state but it was pretty damn close and my first indication that something was wrong was that i felt it in the peripheral vision i saw the world start to drop and helicopter 600 you can squawk at zero four zero five and departure frequency will be one two three point eight five [Music] and that's when i pushed forward lowered the collective a little bit which in that situation as well the shard maneuver probably would have been more appropriate but thankfully what i did worked and then the final error is i didn't just go around i still salvaged the approach and landed and then told myself that was stupid i hooked up to six here towards you squawking zero four zero five and departure frequency will be one two three point eight five zero four zero five let's say i get the free and the frequency will be one two three point eight five twenty three eighty five six hotel that was stupid [Music] take off let me know when you're ready for it take off okay we'll hold position here six here what's up six five four four seven eight point tower turn left and course on way one eight left square for takeoff uh doing other stuff a little bit of a settled from there um might not have been a bad idea just to say hey look today is not going the way i thought let's stop it right here but instead i'd elected to continue and brush it off stay here for a minute so the lessons learned on that number one should have said standby the first time when he asked me uh in the pattern if i was going to 1500 feet that one i'll give myself a pass for i was still high enough that i could do that but when he asked when he gave me the squawk and the frequency inside of landing parameters i mean that's like being in the flare and talking to tower just not a good time to do it not not the right time to be doing that and especially not the right time to be punching in uh transponder codes and taking my hand off the collective so that was should have been a standby absolute standby no questions about it that should have been the end of it right there i was very lucky and that's what it boils down to sometimes the swiss cheese lines up but you're lucky enough that you get out of it and you have you live the tail tell the tale versus it becomes a mishap report and could have been really bad because that would not have been a good situation at all so from there uh i departed the pattern the river run uh went to aima uneventfully hello hotel uh no further traffic observed between you and emma airport raider serves to determine his clock bfr freaking exchange approved good day and six year hotel i'll be back with you here in about five mike you'll meet the squad hotel apartment just keep your squad code and just coming back to airport stick telecom aim the traffic helicopter 536 sierra hotel is right he's planning to the north will be touch and go for payments name of traffic helicopter five three six zero hotels finally the north so to the north dealt back when you threw 200 in the left turn i got there hotel i didn't say outside with flash passing a 300 six zero just north of bama airport and uh stay attention cleared out of the broadway streets out of 1 500. altimeter seven three zero zero seven clear in the bravo at below one thousand five hundred vfr flight following two reserve for six zero two and helicopters hotel roger are you gonna follow the uh river all the way to reserve for uh for american 1194 new orleans tower runway departed ema went to the reserve airport uh they gave me some point outs for some weather made that work it was really big storms around the area but not close enough that they were a factor just still uh obviously within vfr cloud clearances and all that stuff uh going and coming helicopter six air hotel uh moderate to heavy precipitation uh between near 12 and two o'clock beginning in 900 miles perry appears about 10 miles in diameter six here till has that rain inside i got through hotel roger contact approach now on one two five point five twenty five five six year hotel thanks real reserve traffic helicopter 536 the sierra hotels enter the left downwind [Music] runway all right we're gonna go to the intersection here take it kind of a steep approach here [Music] [Music] nice all right we'll take the runway zero traffic after five three six year hotel is departing runway one seven to be a left turnout reserve so oh sorry helicopter 5360 hotel just off reserve passing 900.00 a hotel and uh where do you plan on picking up the river uh six yard towels gonna maneuver over the racetrack here for a second and uh picking the river back up to lakefront [Applause] [Music] remind me of your intentions you're just going over to racetrack anywhere tell just doing a uh low overhead over the racetrack and then back to lakefront rodriguez i didn't know what it was like six zero two woke up there you go all kinds of good places to land [Music] and then coming back so i came back and did a few more patterns tried to shake off that issue that traffic at your twelve to one o'clock about four miles inbound for the field is a helicopter currently eight hundred feet we're looking for that traffic switch hook up to toe that uh the previously departed traffic is at your 12 o'clock three and a half miles turning south westbound 1200 feet sticks your tail look and i'll just send dude [Music] and a traffic that will be following about a mile and a half south of the field enjoying the patterns after six is here hotel traffic you'll be following is just a partying textbook echo making left closed [Applause] ah that's gotta be that's lester option number two number two clear the option tax hey echo six here hotel behind lester you can hear you know it's lester signage your lad takes away foxtrot beam signature six zero tail and then when i attempted to land at signature they had moved these big business jets right in front of my usual path to get to the hangar and i didn't know what to do okay how do we get behind these people lake cirrus one six niner bravo bravo's with you on the r nav for uh one eight right this is one six not a probably from terminal we want it right for the land wins at one zero zero niner cleared the land one eight right one six bravo bravo well i don't know with that fuel truck there [Music] okay [Music] [Music] i hope that truck's in the way all right six here don't like to reposition to uh flight line [Music] there wasn't enough room to actually set down and actually tow it in because that would have put me on the taxiway there wasn't really any room to do anything and rain was approaching so uh luckily i knew lester was flying which you know i had to call an adult and i called put it down i said hey can i reposition put it down on the ground got my phone out called lester fix it to a regular position in this area foxtrot your fox right six you sir that's called lester faster you hear me i'm at flight line hey i need you to get in there's too many expensive toys tonight where you parking flight line yeah where are you at nah but we'll keep rolling for him oh behind me joint golf okay all right see ya golf fox trot to the ramp 160. i don't know why the thing doesn't work at six hotel behind the king air will like to reposition back to uh signature all right roger that you have to hold your position for a sec a whole position six year old no problem but thank you hotel you can reposition for your fox truck got a guy coming off on the uh series coming off on golf holding position okay root position on fox shot series will be hold position six thirds out sanitation uh for one eight kilo charlie lake frontal runway a one eight right flatter overhead before repositioning sorry about that all right roger that so uh i'll tell you what uh you're going to sit it down there yeah set it down here for a second and then i will be back with you when we're ready to go all right oh lester do you see that see that obstacle course is 30 million dollars from the south our parking spot from the south due to the uh park traffic all right six-year-old tail use car support from everybody at your own risk you'll see those requests remain east of uh the long run away for me the winds one two zero eight east of the long runway six zero temperatures sounds a little all right you're gonna be disappearing on me april we're gonna go into the front of the 22. make sure you clean everything the house clear so [Music] [Music] so all right folks youtube man that wasn't me that was him good job just gotta do it look you see this yeah you wouldn't go through that um i came up i probably would have came over like that way or something but i've seen you when he was harvard i came out i was gonna help the bush again and i came out you weren't there and then you called i said lord just pick me up that's a big bro i was trying to call you with the headset but it won't connect i through very good i saw the fuel truck i'm like i can't get through there right it's too windy i mean other than that put about where you were wait about 10 minutes and try it again but i was here i was glad to help this could be done done slowly you know just keep on your lte and keep out of your cell the power you know what causes that good job that was all of you well all right so we got lessons learned uh the biggest thing is don't mess with the stuff while you're in a critical phase of flight that that i know i mean that's stuff that i already know so that's i should already known that [Music] other stuff a lot of rain uh all the other approaches look good this stuff i mean yeah just that's that's a g4 it's it's a hawker i mean there's a lot of expensive aircraft there so i just you know wasn't comfortable with it i don't think anybody would try to taxi through that so i elected to call phone a friend get lester in and then we did uh kind of a very very steep uh approach there but a little bit different format for this episode and the reason for that is i you know i wanted to own up to my mistakes uh this vlog is about the good parts the highlights and the low lights and the stuff that we can learn from you know i've got around 3000 hours and stuff like this can happen whether you've got 10 hours or 3000 hours and anywhere in between these mistakes happen the important thing is that we identify them and can learn from them and own up to them and that's one of those things you know i could have cut this part out and not shown it but i really think that this is something that we can all benefit from and we can all learn from so this is on me something i screwed up something i really internalized going forward after this and you'll see in in future episodes a little bit more of decision making stuff because i think the next episode will actually be more of a debrief too for different reasons and you'll see why uh in the next it's a kind of a
