[Music] it is challenging to manage patients who come in with lots of different problems and you've realistically only got 10 minutes you can forgive them because they've probably waited some time for the appointment and problems probably start building up [Music] prioritizing what's important is the key i personally want to know what is it that they're worried about and what do i need to focus on today and what potentially can wait safely for me to deal with at some point in the future [Music] so i haven't been to the doctors in a while okay so i have a little bit of a list okay have a seat i'm dr ashraf and we've got a medical student sitting in hey yeah nice to meet you right what can i do so the main reason i've come is because i noticed um like uh itching in the inner of my arm and on the side of my breast and the armpit and then he lumps under the arm yeah the arm is the main thing that makes it's like the it makes it feel like that but i don't it's calmed down a lot now okay that's fine so we can have a quick look and see what's going on there okay that's that one the next okay um i have a slip disc in my neck yeah uh i need to have physio i don't know what the procedure is to do that okay that's a nice and easy one i can it's self-referral okay yep and i can give you the number for that yep so that's the number you ring they put in the waiting list and then they start the physiotherapy amazing okay the other thing is so minor there's this spot on my back that keeps reoccurring yeah and i just want to know how to get rid of it so it might be more of a cosmetic thing rather than a thing but does it disappear and come back or it just grows big and small it's a good question i don't know because i'm not okay we'll have a look at that what were the other things you said uh sorry i've got um i've got these um i don't know if they're warped or what i'll take my tries off i can show you yeah and so um i think they need freezing so if it is a wart yeah and what we advise is that you try the over-the-counter stuff first because it's very good um if it hasn't worked then we book you into our minor op clinic um they'll freeze it for cryotherapy yeah okay right let's go around the curtains we'll start with the leg and the back i don't know anything about this i will be honest with you do you want me to just pop you on hold while i inquire [Music] ladies on the phone her doctor surgery has told her there's a pilates class at hall green health center we're doing pilates so what that lady's asking i don't think we do to check with her doctor's surgery good one hello madden thanks for holding none of the staff are aware of this and there's a doctor in the surgery she's not aware either [Music] i wouldn't mind actually i could do it a bit of exercise [Music] yeah they do look like warts so i'll refer you to cryotherapy for that yeah okay let's have a look at the back normally the way it goes is someone will help me plug it so you've got a little sebaceous cyst so um if it keeps getting filled with pus and it's getting bigger and smaller and it's painful yeah when i refer you for the cryotherapy for the legs they can also cut that out okay but if you're happy with a scar because you will have a little scar there when they cut it out so they can get rid of the cyst because it's symptomatic then the itching is here yeah okay just pop your your arm like that i'm gonna have a prod around here i'm just gonna feel for any lumps yeah there we go yeah i mean there's no obvious rash but you do have a little bit of scaling okay now that's fine right so get dressed okay so let's just go through it um regarding the warts and the sebaceous cyst on the back we'll sort that out in our minor rock clinic here yeah so you'll get an appointment for that perfect um regarding the underarm sensitivity so your body seems to be reacting to something does it come out with little hives or wheels yeah i think so yes so it sounds like you might have a little bit of urticaria going on which is like a reaction to certain external stimuli or internal stimuli and it's intensely itchy and then it sort of disappears so what we normally advise is that you go on some antihistamines okay from over-the-counter yeah yeah so if you do that for about four to six weeks yeah just one tablet a day or even just a month yeah just one tablet a day and see if you get that reaction or not it should help calm it down is that everything on the yep that should be amazing let me just double check it yeah that should be everything in um i hope that was 10 minutes it's really helpful for the gp if you explain right at the beginning that you have a list of things to go through so this allows the gp to decide what they need to prioritize how much time they can allocate to each problem and we always try our best to get through as many things as we can perfect okay sorry it was so speedy that's okay that's okay we got through it all brilliant okay thank you um she has um a topic eczema everywhere yeah and fatma go to the doctor and show her your neck okay you know the thing is if it doesn't look as bad as it is you know what i mean because she literally is refusing to move her neck taking a clothes off putting them on anything and how long does it look like this for it started a little bit you know those type of little dogs yeah um since yesterday morning if i'm just touching over that's very tender yeah okay and where else do you tend to get your more badly onto my legs can i have a quick look at your arms as well see how it is at the moment and what creams and things are you using at the moment um i'm using um epiderm all the time hydrocortisone whenever and where it is required but the thing is doctor that she used to get bandages she used to then push them down and like kind of make them a roll and not put them but she's a bit like older now she's sensible and she knows she can leave them there for a while now okay isn't it yeah do you want to try some more of those on your arms and things then just to see if it helps yeah please yeah how old are you now you're pretending nine years yes she's already selecting her birthday cakes no nice to be prepared do you mind me just doing your temperature good girl okay that's normal that's fine let's see okay good fine all right so what i've done is i've stumped through um some of the long sleeve vests for you and some of the uh sort of stockings as well okay i think uh it would be worth trying to use them again if they've helped in the past just to try and get things under control because you sound like you're using the emollients and everything yeah it's just trying to contain it for you i have some free some fluke oxide in the antibiotic as well just with it being so tender and red there we just need to to stop it spreading okay so i think it likely is becoming a bit of a factor there if it's no better in a you know within a week or if it's getting worse rather than better with the treatment then we need to see her again okay thank you all right thank you very much okay bye now thank you you
