this isn't vanity this is a really significant breast deformity that needs correcting [Music] so what's the thing that's really brought you here today my breasts so you've had breast surgery before right I have this would be my third thus far has been a process for me I [Music] but I need the rest of me to come into alignment with it I have so much extra skin it's like elephant skin or something because of the wrinkles and The Dumpling oh my gosh I hate it we can see from the pictures that you have the scar around the areola yeah and the areola has gotten a little bit Yeah dilated well I've always been I was born need rest that's called a tubular breast when the areolas are dilated and the breast base is narrow tuber being a potato it's the Greek word for potato and I'll bet she had sweet potato bread oh my goodness you're actually you know you really are freaky I just know what these new cans and a new nose let's talk about the nose this is news to me because Tiffany did not Mark anything on her paperwork indicating that she wanted a nose job so the thing that concerns me about my nose is that my nose leaks a lot so basically you're telling me it's called rhinorrhea so you're saying I know that's what it's called I mean well examine you know we'll take a peek it's very sensitive to the air and if it gets cold I'm just leaking leaking leaking so I'm hoping that the leaking will stop okay so the perfect distance between the clavicles to the nipple is 21 centimeters you are 32. so you're 11 11 centimeters lower than you should be Tiffany's previous doctor went too far she has extremely large breast implants with very stretched out skin and a lot of droopiness you can need a lot of surgery but I will tell you the more I have to raise you the greater the amount of tension around the implant and the greater the amount of tension the greater chance the scar will widen okay and the greater the risk this is not an easy case Okay this isn't vanity this is a really significant breast deformity that needs correcting I'm not gonna lie I'm I'm a little bit nervous but I'm confident in my decision I've always been a little fake on the exterior but real where it counts which is that heart yeah my heart is the only real organ in my body look at the hair dude get off my hair get off my hair it's really you there oh shut up this surgery couldn't have came at a better time in my life I have some good offers on the table and I know that these two doctors are going to just give me the extra Edge that I need to be greater than before okay so I'm going to start the rib and then we'll do the handshot at the same time so we can rock and roll when Paul and I do simultaneous surgery it's a very complicated Orchestra of moves in which we have to make sure we both enhance and not interfere with each other but let's make sure we give her a good operation she said some crappy surgery in the past yes me the plan for Tiffany is to start by harvesting rib cartilage through an incision under the breast then while I'm using that to build up the bridge of her nose and fix her deviated septum Terry will be swapping out her implants and get giving her a breast lift so I'm going to start now well right through this Garden so at this stage we were just getting down to the top of the rib looks like it's a little bit calcified when we use the word calcification what we're saying is that cartilage which is moldable or bendable becomes very hard and we can't carve it we need to be able to carve cartilage to put it inside the nose unless that's bone right there how it goes the chronologic bone is actually a little bit more medial than we thought the longer I stand there and the time is tick-tocking away the more I realize the less time I'm going to have on her complicated breasts because there's a limit to how much anesthesia a person can tolerate in one session it doesn't feel clean coming off you don't want to keep a patient on anesthesia too long because all it does is increase the risks of something bad happening how's it going okay so the rib is coming out right now and I had to go a little bit more laterally to help our lateral as you have to go seven centimeters could you do me fear could you measure the width of the breast about 14 and a half or so okay so you had to lift up seven of that 14 and a half that lower blood supply might be impaired you know as a surgeon I'm having a bit of an oh crap moment Paul did more than I anticipated that risks the blood supply to the nipple so I'm concerned not to mention the anesthesia so you have how much longer on the nose about a good four and a half hours I have about four and a half so that would take us over eight hours of anesthesia and there's the potential problem of the blood supply to my nipple it's not happening today there's not going to be enough time to do a good job on both the breasts and the nose it just makes the little hairs stamp the back of my neck we're used to stressful environments and surgery but we always have to keep our cool and do the right thing for the patient let's not do it then the thing is when she wakes up we'll have that discussion with her a major chapter in my life a few weeks after I have my rhinoplasty I went back in and got my breasts done every ounce of pain was worth it I feel like there's nothing that I cannot do and I cannot wait to share that with my friends [Music] I for one am so excited to see how she looks at the end of the day I just want to see her happy because you know what I just want the insole to match the outside you know agreed thank you oh that's my girl right there seeing everyone's faces and how lovely Jaws have dropped it's like the world to me hold up hold up hold up hold up where's yes when you get your breasts done you're really supposed to have them replaced every sort of 10 to 15 years implants that are more than 13 years old have an 83 chance of being ruptured these implants being from the 70s who knows we may take them out put them on there there might be streaks of cocaine still left on them I feel like that there are particles that are rolling around like the size of edamame the good news is you were done the old-fashioned way which is to put those old stop saying old those uh antiquanticated or Antiquated you had the other silicone implants put in on top of the muscle that was the old that was the other way that was done previously theoretically uh we would take those implants out and then lift up your pectoralis major muscle peeling it off the ribs and then lining the inside of the breast pocket with them so that the new implants go under the muscle these implants are forgive me don't say old ancient I need them out yeah you need them out look at you you look amazing it's one day out of surgery look incredible I hear what you're saying but it's not it's not Computing how are you I feel that I should have been left longer at The Recovery Place where where's your dress who took your dressing off I did what is this the drain from my you took your drain out kill me this is so dangerous Janice oh my god well I'm sorry so sue me I asked her no I'm not going to sue you okay it's not about you well the problem with this is this is a sterile thing that goes inside so it's still sterile no because this is once you disconnect the tube from the bulb you've contaminated the entire thing and bacteria can crawl up the tube into your breast pocket and cause an infection I've never had a patient do this before are you trying to have a disaster on me don't do that no look at all that fluid you're doing things that could turn into a big problem you didn't tell me not to take it off right that's true but I also didn't tell you you know not to jump out of an airplane but you're not going to do that either right oh my God all right enough in my 25 years of being a board-certified plastic surgeon Janus is by far the most difficult patient I've ever had is her breasts worst enemy I'm gonna wrap you up so that no matter what you do you can't touch these incisions anymore it's not the decisions I I did it on I did it on you just touched the incision in front of me what those yeah those if you would give me something for the pain doctor please you just spent three days in the hospital on IV Dilaudid most people are on aspirin by this point I don't give it about most people that's nice for them you're gonna have to give me something I'm I'm serious you know I I don't want to feel this bad the ex attic is coming out and we've got post-operative pain management problems I'm kind of at a loss how to deal with her right now it hurts to blow my nose it hurts the ticket I mean a lot of Agony so I want to be prescribed for two or three days the same that I had at the hospital this is classic drug seeking Behavior so let me tell you why this one hurts more than that one do you know you had those masses up here I took one of them out it was like a calcification like the type you have when you have breast cancer but it wasn't a breast cancer it will not be a breast cancer it's just a reaction to the little micro rupture of the silicone okay thankfully they're benign they needed to come out but they're benign and it's all gone so thank you so I have brought you a fresh bra that I'm going to put you in this is it black black surgical bras yeah no this isn't the Victoria's Secrets lingerie catalog this is recovery get it together but I mean you're gonna go out yeah I have to work you're going out tonight of course with drains in place she needs to be saying she needs to show up with drains and narcotics on board you had four and a half hours of reconstructive surgery can I talk turkey with you for a second can I be a little uh can be a little brutal might be a little brutal you're gonna this up if you keep acting like this you're gonna pick it up you're gonna get a complication and you're gonna lose both breast implants I'm not kidding no recollection of last week at all nothing look whatever I said however I behaved I you know good news is very sorry it's not me it was the meds it just was the meds I'm sorry it was the meds I didn't know if I broke my sobriety it doesn't count because I was under doctor's orders well he had surgery I mean yeah you went through a lot that was amazing we went through a lot what did we do you were a little bit discombobulated that's a good word I'll go with that will you breastfeed as a child you know what I was impressed baby that's why I'm fascinated with my I knew it well you know when you're just coming into your own as a kid yeah all I saw were breasts where everywhere every girl is just please just like a breast freak so I became [Music] over here big guy you know what's cool you had some redness at the end of the procedure and it got worse over the next few days just because of all the surgery I did it's gone you are surviving your up my nose my nose is non-functionable but pretty I can't breathe I'm not getting circulation I'm always having sinus infection so I it up you may know me from The Real Housewives of Atlanta or Married to Medicine I'm a guru of beauty and style I was born in Atlanta Georgia in the heart of the ghetto my mother was very religious the presence of God and our household was everything growing up I was given a basketball a football I never wanted to play with the balls those kind of balls at that time as a kid I did not like my nose it was wide it was long you could drive a car right up in it once I got some money the very first thing that I did was run fast to the doctor when I got my first nose job done I should have left well enough alone but I didn't vanity took over my second nose job was probably the biggest mistake of my life I love the shape of it but it's not functionable most people breathe through their nose and I've had to be creative I read through my not being able to breathe I dealt with it for 30 years but now I'm ready to move forward to fix the problems that I created no I don't want a big fat nose again but I like to put all of that to rest bottom line is I would love to have a functional nose [Applause] gray day Dr NASA Dr Dubrow here is Mr Eubanks and his mother how are you hi pleased to meet you Jerry Dubrow nice to meet you pleasure Dr NASA what's your name hi I'm Queen your name is Queen Queen Elizabeth Queen Elizabeth can you imagine that how are you see what I have to deal with it Dwight Eubanks I thought I'd bring a little libation to get the party started yeah so it's a fine wine to really give you more essence of who I am and what I'm trying to do and hopefully you guys can do something to me to make me more fabulous than what I'm already in Dwight walks in he's dressed to the nines he's a Dapper from just like looking in the mirror so Dwight your rhinoplasties rhinoplastis you know it's always been a thing for me I did not like my nose so the first time it was really fine I should have left it alone but I just want to test a little bit smaller complications okay the breathing the breathing the result the breathing okay just by looking at Dwight's nose I can tell that this nose has been pillaged everything has been taken out of it to make your nose more functional so you can breathe better we have to reverse what you had done so you can make it bigger I have to can't you just cut that and open that no the problem is you have to put some have enough tissue there okay so I think our next step is to go in the exam room and let's take a peek let's do it okay let's go I started out with the huge nose and now we're talking about going back to being large large sometimes can be good but I don't want to go back that way so let's try to be as gentle as I can I'm a virgin this is called the nasal speculum what are you going to do with that I'm going to gently try to insert this in your nose this is what you call a virgin nose see I mean it's tight so he's completely overdone so let me just do this right here you okay am I hurting you a lot I'm dying oh my goodness yeah I can't even get that in there he has no air going in there's no Airway they took out a big wedge of nostril too much so I might have to fracture re-break this bone here in addition I need cartilage and taking a little piece of rib to rebuild your nose with it's difficult complex high risk surgery and you have to be prepared for that well between the two of you I'm confident that it's going to work as long as we don't go back to huge I'm okay with it what what can I ask for yeah right two good looking man at one time there you go look at you all fabulous who designed these clothes this is horrible I can't wait to be able to breathe it has to be a much healthier life for me this is horrible what's going on with this headpiece this has been 30 years of suffering so I'm forever grateful you know for this opportunity I've never seen anyone put it I think oh my God you know I'm ready I'm ready to go I'm ready to go so how you feeling I read here hungry so am I I'm hungry also I can just reach up the Hogs and pull out a ham sandwich I'm so hungry see you soon Dwight's nose is extremely botched so what's Difficult about this surgery is we have a fine balance between function and Aesthetics for Dwight's surgery my team and I will Begin by harvesting cartilage from his rib and ear composite grass made out of skin and cartilage at the same time next we'll straighten the nose by fracturing the bone and adding cartilage to build it up then we'll use the composite grafts to expand the nostrils and open the airway my old nose held me back from the simple things in life being able to stop and smell the roses the day after surgery I could breathe it was a domino effect I had more energy I can think clearer food tastes better that oxygen just makes a difference in your life it's like being baptized dipped and the holy water and been made new the old nose is dead we have to bury it but not in sadness but enjoy the only requirement that I gave my guest was to wear black [Music] [Music] all right we gathered here this morning not to moan and not to be sad but just give all the praise and the glory for those that never worked out with the old and in with the new wow [Applause] what can I say about my baby he was beautiful then and he's beautiful now [Music] since Dwight's surgery everything has been different his breathing he's sleeping he don't blow his nose like he used to now rest peace before my surgery my nose was so tight nothing can get up it and nothing could get out of it now that I've had the surgery done I can breathe I can smell I taste things differently I can feel a difference and that one difference was not physical appearance but the ability to breathe the Lord taught me a lesson about vanity and boy did he give me a weapon I'm forever grateful for this whole process it has
