He doesnโ€™t say he doesnโ€™t do it anymore. He just uses it more sparingly to subtly balance features/make a face proportional. He gave examples of the chin, temples and tear trough of which he still uses filler for. Heโ€™s against overfilling/coming back for more and more and piling it over old filler causing bulk. Also where does he mention Botox?

so if a slimmer face is generally more attractive why is it that we are using dermal fillers to fill the face assuming that you are not too skinny and gaunt in the face to make you look unhealthy generally slimmer faces are better looking to illustrate this point we're going to look at some celebrities who have put on large amounts of weight to play a certain role and our first example we have Christian Bale now in his roles as American Psycho and Batman you can see he is very fit he's got a slimmer face but then he goes on to put on a significant amount of weight for his role as Dick Cheney in the movie vice and you can see he's lost all his definition of his face he looks round he's lost his cheekbone definition his jaw sort of blends into his neck let's take a look at Charlize Theron what a stunning beautiful lady and she has also changed the way for different roles you can see here in her role in monster where she's gained a significant amount of weight and she again has lost those cheekbones that contour the face that beautiful contour that she had to her face it's all become a bit more round and she's purposely gone to lose a lot of appeal by putting on this weight and playing this character many people will argue that as they age they tend to lose facial volume and therefore they should replace that volume with with dermal fillers but it's it's quite often the case that have patients tell me that they actually like their face as they get a bit older so maybe in their teens or the 20s they felt their face was round like cheekbone definition as they got older they lost a bit of facial fat and interestingly they feel their faces look more attractive and they look better as I go into 30s and 40s let's look at this example of Jennifer Connelly I think she's an absolutely stunning looking actress and because she's been doing movies for quite some time we can go back to her earlier roles and you can see that she's got a lot more of a baby face back in her younger days but she actually I think she gets more attractive the older she gets and you can see she's lost a bit of facial volume but it's actually improved the definition of her cheeks and highlighted some of the contours of her face and you can really see that she looks great despite losing some facial volume so again why are we using dermal fillers to inflate the face if a slimmer face looks better especially when you see so many overfilled faces out there there's many examples well some people argue that they're putting in fillers to define the cheekbones to define the jawline however fillers can't really do this now you will probably see a lot of before-and-after pictures on the internet where people have shown you a defined cheekbone or defined jawline but the problem is that these photos are taken immediately after treatment so you're not getting a good picture of what happens long term now my belief is that fillers tend to flatten and spread a little bit with time so what you put as a defined line in the cheekbone or in the jawline eventually becomes pushed down it's flattened and spread so you cannot create definition with Phillips I've had patients have multiple multiple mils of filler to try and create this defined cheekbone or jawline look only to end up with a puffy face and we have an interesting comment from our channel here which says I know a guy who had 15 mils will fill out every bit of his character and his once handsome face has been plumped out and smoothed out he's happy but he looks like an egg the only way to get defined cheekbones or jaw lines is to be at a certain body weight or fat percentage level that they're fat over the cheekbones and jawline is thin enough to show the definition of your bony structure but it can also go too far and that's when you if you get too skinny and you start to sink in and the cheeks sinking the temples sinking around the eyes that can take things to the other extreme which is also unattractive so you could why don't you use dermal fillers in the case where someone has become too slim or has got too much fat loss in the face and the reason for this is fillers and generally not great at fixing global volume loss that means you have to put in filler in virtually every part of the face and this requires a lot of volume of fillers and it's really unfeasible in many ways so in this case it's much better to simply get back to a weight which is more healthy and you will not need to spend a cent on fillers so if we don't use fillers for filling the face what do we use them for well primarily fillers are used to balance and proportion the face they're there to balance up your features and say for example if you have a small chin to enhance the chin if relative to the cheeks your temples are a bit flat it's the balance the volume in the temples with the volume in the cheek so it's not for generalized feeling it's for balance and proportions and the other thing we use fillers for is to smooth transitions so for example the tear troughs the transition from the lower eyelid to the cheek is often seen as a tear trough and that's a hollowness just along the lower eyelid and this can be filled to help smooth that transition but only tiny volumes are needed so we're just smoothing things out so the key message here is don't use bulk volumes of dermal filler to generally fill the face that is every drop of filling you place in your face should have the end goal of making you look more attractive your injector has to have that in mind when they are injecting you otherwise you're just Bob filling the face for no particular reason increasing its volume without any goal in mind and it only takes the tiniest smallest amounts of filler to make a really nice change to someone's overall face and their appeal my most common volume of treatment is actually one Miller filler and I place this one mill very strategically in different spots in the face to support to balance to smooth transitions and that can achieve a really nice change you can get a really big change with the smallest amount of filler and at last for so long so if you are thinking about having feel are done or getting filler done make sure your injector carefully assesses your face prior to each dermal filler injection there should be a very clear aesthetic goal in mind for each treatment and if there's already enough there don't put any more in and also don't be afraid to actually take out dermal fillers if they don't look right don't expect derma fillers to dissipate by themselves they generally take a very very long time to go away one of the best ways to maintain an attractive looking face is to maintain a healthy weight to give you that nice contour and slimness to the face and if you underweight make sure you try and get that weight back on and that will bring about a lot more attractiveness to your face rather than simply just putting in fillers
