What to Expect in the 1st & 2nd Trimester + Top Tips for Surviving from a Pregnant OBGYN

hey this week we are talking about common first  trimester symptoms how to beat them some second   trimester um things to expect all coming from  this mama going through it for the second time   so it's kind of our first trimester Vlog part  dose yeah you may have caught us the first time   through pregnancy when we went week by week  through pregnancy yes kind of detailing all   of the changes that happened throughout that  this time we're doing pregnancy with a toddler   running around so we haven't been able to be as  week by week through it we wanted to catch you   guys up on how things are going for us and  let you know the things that we've learned   along the way now that we're doing it for a  second time yes if this is your first time   meeting us I'm Sarah I'm a board certified  OB GYN mom to a toddler with baby number two   on the way and I'm Kurt I'm a board certified  pediatrician and we are the doctors bjorkman [Music] welcome back as we mentioned this week is a vlog  kind of hybrid combo going back through all of   the things that we were the first time through  pregnancy and had to relearn the second time   right what to expect with that first trimester  um and kind of transitioning into the second   um these things did not come as such a surprise  this time around um but if this is your first   pregnancy um want to prepare you for some of  the very common first trimester symptoms that   you may or may not experience depending on you  and your pregnancy and then give you some tools   yes tricks tips as well as sharing you know some  things that have been different between the first   and second pregnancy as well as what some of um  you mamas out there have shared that things that   were very different between first and second  pregnancy so to kind of get started let's talk   about very very common first trimester things  so when you think about being early pregnant   that one of the things people think about is  being nauseous morning sickness vomiting really   not feeling well and this is really common  more than 80 percent of women experience   um some kind of nausea or vomiting during  pregnancy we have an entire episode all about   nausea and vomiting and pregnancy so definitely  check that out if you find yourself in that boat   um but tips for morning sickness number  one morning sickness is not the right name   um kind of all the basicness maybe you're just  nauseous at night it really just depends on your   flavor of nausea but it can be really really  brutal you can feel pretty miserable you can   be racing to the trash can in the middle of the  day um so things that I found helpful other mamas   find helpful in the science says is helpful  so small frequent meals throughout the day   um even having some Saltines by the bed so  that you can eat some crackers before you   get up and going in the morning and then kind  of keeping your stomach full throughout the   day not full full's the wrong word but keeping  some food in your stomach throughout the day   really seems to help people Studies have shown  that protein Rich meals food seem to help more   um so peanut butter nuts protein bars things  like that are what the data shows my experience   was Carby nibbling on carbs all day long  seemed to help keeping some Goldfish in my   pocket I feel like you know first pregnancy  I'd eat a kind of a can of Pringles at night   um and that seemed to help me a lot but just kind  of try to keep up with those small frequent meals   and snacking throughout the day if you need to  keep a bagel in your pocket no big deal and I feel   like the other big piece there is like especially  if you're having MSS actually throwing up as   staying hydrated it's really really important  yes yes and so keep in mind if small frequent   meals some of the other things we talk about  in that episode are ginger ginger tea ginger   lozenges sometimes sucking on like sour candy so  a lemon flavored candy or peppermint seems to help   patients as well so those are some things you can  try um if that doesn't help know that there are   safe prescription medications that your doctor  can prescribe you that are safe for you safe for   pregnancy and make it so you are not so miserable  all the time and as Kurt said if you're throwing   up so much that you can't keep liquid down you're  not eating you're losing weight you need to touch   base with your doctor because you may need to  go to the hospital to get some IV fluids and get   that under control yeah and so while some nausea  vomiting is kind of very expected yes like if it's   anything more than just a little bit like talk  to your OB GYN like there are things that they   can do and having a great OB GYN can help you out  in terms of making it through that first trimester   totally on that note you may luck out and be one  of those people that just does not feel nauseous   at all lucky you don't tell your friends they'll  be very very jealous but that can also be very   normal don't be alarmed count yourself as one of  the lucky ones because it can be totally normal do   you feel like there's a difference in that nausea  you felt between first pregnancy second pregnancy   better second time around maybe um better second  time around with in pregnancy number one for those   you know like week seven through six through  ten ish I feel like at night I was just kind of   low grade nausea constantly and thus constantly  eating um to try to help this one I don't know   if it was just it was all so different it was an  IVF pregnancy I got pretty sick after my retrieval   um and so it was it was all just very different I  don't know if I was just so feeling unwell overall   from the start from yeah um but did not have  nausea really there's some like folklore that   um boy male pregnancies not as much nausea there  is actually some data that female when you are   pregnant with a female morning sickness may be  worse so I don't I don't know um but I dodged   that bullet a little bit this time now I feel  something that you've had with both pregnancy but   was maybe more so with this one was fatigue do you  feel like that's a safe assessment a really common   first trimester symptom is extreme fatigue extreme  fatigue um and I get maybe it was less surprise   Rising this time maybe with the first pregnancy I  felt like I could take a nap when I needed and or   wanted to with the second pregnancy and the  toddler less easy to just nap when you want   um I also was studying for my boards and  so it was a recipe for just really being   tired all the time you're also moving which  wasn't fun yeah so super tired super common   um rest when you can if you are coming home and  feel like you are ready to go to bed at 7 pm   that's okay enjoy falling asleep on the couch this  too shall pass energy really starts to return kind   of around the second trimester so give yourself  that Grace know that it's about balance if you   feel like you need to rest rest your energy will  return if you want to get back to working out or   just being able to stay awake at night it's coming  but know if this is your first pregnancy you may   be super super tired and that's very normal yeah  and I think it's a good reminder for the partners   out there too to just expect some of these things  and just wean on like allow your partner to lean   on you and be supportive of them I think kind  of knowing what to expect is really helpful as a   partner too because oh this is part of pregnancy  here's what can I do to support this and to be   proactive a little bit helps yes you are growing a  tiny human it takes a lot of work and energy other   first trimester things that are pretty common  for patients to experience our breast tenderness   um some people do some people don't but in  those often in the early when you're taking that   pregnancy test in those early weeks you notice  oh I feel like I have some breast tenderness   maybe some bloating maybe more sensitive to  Smells may even notice food aversions early   um maybe really more sensitive to heat and  so these are really common I know in my first   pregnancy I was very sensitive to heat I couldn't  even like you know high-waisted leggings are just   a dream and I remember not even like being able  to have my leggings up over my abdomen because I   was like I'm so uncomfortably warm and so it was  really sensitive like would feel pretty nauseous   if I got too warm and so some people all of a  sudden don't can't handle their coffee anymore   don't like drinking milk and so the some people  are like my sniffer got so much better when I got   pregnant they noticed this increased sensitivity  to smell so it may be also something you notice   and then other things you may notice are changes  in your bowel habits so all of these things are   kind of partially due to a rising progesterone so  as this pregnancy is developing your hormones are   starting to increase progesterone estrogen all  of these things progesterone so progestation is   the the guilty one with a lot of these symptoms  it tends to slow everything down it slows your   bowels down it slows your digestion down so you  may be constipated some people notice the flip   side of that where they have more loose stools  so again different flavors of first trimester   symptoms and you may notice some gerd heartburn  because again that progesterone really slows down   your gut motility and you may find that you  have increasing heartburn so a bowel regimen   being aware of this is a great idea Miralax Colace  yours tool softener of choice go for it very safe   in pregnancy as well as some Tums or something  help with that acid reflux if you notice that   um those are kind of some Pro tips for that other  things that patients ask have questions about   all the time cramping bleeding what is normal  what is not um and how do you navigate that so   the thing I always you know encourage patients to  remember is you have this tiny little embryo that   has to burrow its way into your uterus and form a  connection between your maternal blood supply and   little fetus so there is going to be some bleeding  at this interface between embryo and uterus okay   and that's normal and often it just gets resorbed  and stays or stays behind the pregnancy and you   don't notice it and you don't have any spotting  but up to a third of pregnant patients have some   bleeding in the first trimester and it can you  know I say this is never normal but it's really   really common one of the most common causes is a  subchorionic hemorrhage and we will have a video   coming all about that but you know very common  especially in IVF pregnancies I had a small one   did not know that I did I didn't have any bleeding  but when I had my ultrasound sure enough could see   this subchorionic Hemorrhage and so it's just  a scary nerve-wracking thing so a little bit of   spotting very normal bleeding like a period you're  going to want to touch base with your OB GYN not   that there is anything you can do about it at this  stage of the game but if you do have significant   bleeding you would your OB would want to think  about giving you a medication called rhogam   depending on your blood type if you are like an  a negative RH negative blood type and you have   bleeding during pregnancy rhogam is something that  we give to patients so something to think about   very nerve-wracking check out our episode coming  up soon um what do you think I've been things that   are particularly helpful for pregnancy what are  the things you did the first time that you wish   you were doing better this time or things that you  didn't do well the first time through that you're   doing now are putting a focus on um second  time through I think the first time through I did not exercise as much as I wanted  to knowing the benefits of exercise and   wanting that to be a goal and was very hit  or miss and I really was hurting at the end   like 20 to 34 weeks was kind of rough in  terms of pregnancy pain so SI joint pain remember all those little back massages  you'd give me and I had my little belt   um and then after that kind of got better I think  around 26 or 27 weeks in the first pregnancy then   I started having synthesis pubis dysfunction so  SPD pain um where it was like hard to put on shoes   or roll over in bed or you know walk upstairs and  so this time I was like I need to be stronger I   need to keep moving I want to do more exercise  and it hasn't like been tremendously more but   it's been more um and I think we've been making  it even just like walking more regularly yeah of   some of that's probably chasing the baby around  but um like you can't not it's not you can't not   be moving I feel like when you have a toddler  um and so I think that has been a different   I have fell and we'll talk about  this in a little bit some other   um differences people experience in the  second pregnancy um are just you're busy here   um or at least we have been and so some of the how  many weeks along are you and I'm like I don't I   don't know is it 16 or 17 or are we at 22 or in  the first pregnancy you're like I am four weeks   and six days today like every day every week we  took a picture you know it was just it was funny   um but just you do know a little more what to  expect you are busier so it has felt like it's   gone much faster this time um the other thing  I think there's there's this piece of like   nothingness really is a surprise as much in terms  of like oh you've been through it but also things   are still just as like amazing and wondrous  so like there's a little human growing in her   belly and like you know I think as Dad it's been  just as much fun to like and I put my ear on her   belly and listen to the heartbeat that we were  able to hear kind of starting a second trimester   and still like getting to feel kicks and like all  of that joy and I did feel kicks way earlier this   time I feel like with the first pregnancy it  was around 17 weeks My Placenta was in the back   um and I thought that was it was pretty early this  one I started noticing it like like 14 weeks I was   like what is that what is that and I was like  oh I think that's fetal movement and I had an   anterior placenta which can mean you often don't  feel it till much later but weirdly felt the baby   moving sooner and I think that's just because I  knew what I was feeling for this time so that was   kind of fun it seems to be a bit Wild so um we're  hoping that doesn't necessarily translate after   hoping for a more chill one this time we will see  I don't think you get to order that but I'm hoping   um another first trimester questions patients  often have is about weight gain a lot and a lot   of this weight cane what can I eat what kind of  meds can I take we covered this in a lot of detail   in our episode kind of called the do's and don'ts  of pregnancy which we will link here now what yeah   yeah no it's a great really comprehensive episode  um and a lot of how much weight gain you expect   kind of depends on where you start beforehand  right it does yep so if you are a normal BMI   when you start pregnancy we kind of recommend  you gain kind of 25 to 30 pounds or so if you   are underweight then we want more than that you  know 25 to 35 pounds if you are overweight we say   more like 8 18 to 20 and if you overweight is BMI  25 to 30ish is that and then if you are kind of in   the obese category we say 11 to 18 pounds during  pregnancy and when does that weight gain happen   so it's variable um and so something to think  about is during that first trimester you may not   gain any weight and that's okay you will get there  um you may gain some weight in the first trimester   you may have a lot of morning sickness and need  to be constantly keeping some food in your belly   to feel better you may be doing IVF and often  patients on IVF meds um gain some weight whether   that's from some fluid retention from all of the  hormones from um extra just needing more calories   to help you know coping mechanisms sometimes IVF  patients find that they gain weight as they're   going through stem and so it all balances out I  do tell patients that you know in the second and   third trimester that weight gain really picks up  a little bit that is when you your body needs more   calories 350 to 450 extra calories a day in the  second and third trimester but again that's not   that much that's a yogurt or you know a ratio a  cheese stick in a granola bar so um and I think   not all the weight ends up being Mom weight either  right like the baby there's baby weight there's   amniotic fluid weight also in your body's blood  volume increases breast volume all of the things   um and so we really we do break that down in that  episode so don't be alarmed if you don't gain   weight if you find yourself losing weight because  you are so sick again talk to your OB because we   want to you know make sure you're you're getting  enough fluid your electrolytes are normal and we   don't want you suffering um because that can be  pretty miserable last but not least kind of other   question patients tend to want to know is like hey  what meds can I take during pregnancy what's safe   I have a headache I have you know I'm constipated  what can I take so um we generally if you need   some kind of pain medication roll your ankle have  a headache we say start with Tylenol we recommend   not taking NSAIDs Motrin ibuprofen during  pregnancy and the big reasons for that especially   the later in pregnancy second third trimester can  potentially affect kidneys or developing baby and   also a special extra blood vessel that they just  depend on the PDA yes if you mate your doctor   may be prescribing aspirin for you there are and  that is there's great data that it can help with   preeclampsia prevention in certain patients if you  have other blood clotting issues your doctor may   have you on aspirin and that is very very safe  so again follow your doctor's recommendations   but no General pain medication that is our go-to  is Tylenol also so definitely want to make sure   you're taking your prenatal vitamin this is  a great one to be on you know before you get   pregnant but if you find yourself pregnant now is  a great time to make sure you are on that prenatal   vitamin that has at least 400 micrograms of folic  acid there is a whole debate about folate versus   folic acid all of the studies that have been done  thus far about prevention of neural tube defects   have studied folic acid and the number for that  is 400 micrograms it is in most standard prenatal   vitamins so just check do you have a favorite  prenatal vitamin how does someone find a good   one whatever is maybe you have some insurance  coverage for something that's a great way to do   it whatever your grocery store carries and then if  you you know some people can't tolerate vitamins   you're nauseous it's the first trimester sometimes  chewy like a gummy prenatal can be very helpful   um those don't have iron in them so if you find  yourself needing iron during pregnancy you'll   have to switch back um I have used what have I  used um and for mom I've used ritual I've used   whatever the drugstore had all have been fine I  found some of the one of the maybe I did a one a   day for a while that I felt like I was burping up  vitamins so that was why the the ritual has this   like lemony aftertaste so that was better um but  definitely that one I think costs more than most   the others you've tried it does um there's  some very very fancy um prenatals out there   um not necessary as long as I've got 400  micrograms of folic acid yep um and something with   some Omega-3s DHA is also um a good thing to look  for so just thinking back now after this first and   second trimester what are things that you have you  noticed anything different anything been different   for you this pregnancy dear um I think you know  I as I already mentioned the big things are like   just they've done it before and so there aren't  as many surprises or the but like it's still yeah   just as exciting then just you're busier you're  busy you're busier because you've already got a   keto on the ground that keeps you occupied yeah  so there are some really special things you know   about it being a first pregnancy and and a little  bit of that anxiousness and having time to read   and and do all those things and that is a really  special time I pulled our Instagram um Community   to say hey what was what I shared this mean which  we can put up here of like differences in planning   for the first pregnancy versus second baby and  um I found it was very true with the first one   was much more detailed yeah analytical reading  doing taking bum picks doing all these things and   with the second one it's like man we're gonna  have a baby here we should get ready for it   um and so I asked the IG fam you know hey what  are things that you found different between your   first and second pregnancy and resonated with  a lot of these things someone said LOL I forget   that I'm pregnant and that is certainly a very  real thing you're just like I got things to do   babies in pulling all the toilet paper off the  roll I you're just busy people said you know I   didn't stress as much about the little details  compared to the first because I knew what to   expect someone mentioned I no longer nap whenever  I'm tired or need a rest feel that so deeply um   some people said less nausea less stress you know  you know what you're doing and some people said   it would nausea was way worse someone did mention  first trimester with a girl was horrible compared   to boy so there's there's another interesting way  less bum picks way less no time because there's   a toddler so far it sucks exponentially more  so you may get the flip side that your second   pregnancy is way worse um little things don't  matter as much I think that's a time for it   um this person said I'm only eight weeks with  number two but more body aware you know which   I found I noticed those kicks a lot earlier  this person said more energy sick more but   not not as strict about avoiding lunch  meat so there's just some things that   you're very hyper aware of in the first pregnancy  which is wonderful like give that to yourself it's   so fun it's it's an exciting and terrifying time  terrifying time um a lot bigger a lot faster and   achy sooner so some people do notice physical  body changes sooner your core is just not always   as strong as it was the first time around and  so you may notice that you start showing sooner   your Bump is a little bit Fuller sooner um I  definitely gained weight sooner in this time my first baby was perfect and I wondered  how I could possibly hope for that two   times in a row that's my mom and  I'm the first baby so thanks mom   um less naps yo yeah yeah yeah I forget which  week I'm in um I don't care about the nursery   being ready oh that a hundred percent um I think  also just like the perseveration over what you   need to bring baby home is also nice in terms  of you know we we talked about like packing a   bag for the hospital and what to have in your  nursery and did all these things 32 weeks and   still don't have anything set up Nursery  was done it 20 weeks with the first baby   um and so some of that is like you have this  stuff from your first kiddo so you don't have   to do as much and you kind of know the things but  also like you know I think maybe that's something   we can do in the upcoming weeks in terms of like  what are the things we found really important for   a hospital bag right one of the things that are  really important for the nursery right haha poor   second child that's all I have to say because  that first one gets all of your new things yes   um less planning less bump picks didn't even have  the car seat installed when my water broke so much   more tired parenting a toddler older depressed  all of the above someone asked me how many weeks   I was today and I honestly did not know yes yes I  feel that I think I've taken one bump pick whereas   last time went week by week so we're gonna try to  go week by week with the baby with baby this time   for all of you and to document that so whether  this is your first pregnancy second pregnancy   eighth pregnancy wherever you're at you're at  thank you for joining with us as we go through it   the second time yes we are so looking forward to  sharing more pregnancy content in the coming weeks   um getting ready to go back to the hospital  and have another baby setting up a nursery   here before this kid shows up um as
