christmas is coming to fitzpatrick referrals and for the staff it's a chance to have some festive fun merry christmas what do you think of that yeah what is that what is that there are some mince pies up here go for it [Music] for the patients who have to stay in over the holiday there's also plenty of christmas cheer our long-term patients get advanced calendars so they can count down to christmas which is always good fun oh they get special breakfast in the mornings and then they get a present all wrapped up they enjoy much that we do they get very excited because we're all like yeah christmas and so they're exactly the same happy christmas i love christmas i absolutely love christmas it's just the best time of the year [Music] what are you saying one pet who enjoys the winter weather when it comes is adam a 48 kilo alaskan malamute adam leave it he's part of a pack of six sled dogs owned by colin and andrea and at weekends they love nothing better than to race with them across the british countryside we'll be out three four nights a week training and friday we'll go away to wherever in country the race is out i don't think there's a greater adrenaline rush than racing through a forest being pulled by a team of dogs so i'm coming at you really it's a great great experience first time we went racing we're in the middle of a forest and it was pitch black and about 11 o'clock at night and all i could hear were hundreds and hundreds of huskies howling it was really surreal we sound like wolves so it's beautiful we do love them [Music] today though colin and andrea have driven over five hours from blackburn in lancashire to bring one of the dogs back to see knoll and this is adam this is where we've come here and it had him adam's um just turned two he's a malamute and he used to love running around not so much now if we take him for a walk after like 20 foot adam has to sit down and so he just he just can't play like he wants to and he is playful he's a kiss yeah a year ago adam tore the cruciate ligaments in both knees and despite his local vet's best efforts he's still in pain when he walks we went back to vets and really there were nothing more he could do for him right from put him down or give him new knees here we are getting new knees for adam i'll put him down so adam bridgehouse please andrea colin and their dog helper sammy are hoping noel's expertise will finally get adam walking again hello mate how are you today how are you talking all right i'm not talking today not talking not talking today you might give him up for me and i'll check him again yeah good boy adam all right just hold up now here we go buddy the kneecap's slipping pretty bad now yeah you can feel it grating also with that today noel will fit the first of adam's two new knee replacements oh dear oh dear both of them are gone downhill since i last saw him what i'm going to do today is not an easy job and you need to understand that there are potential complications alignment is a big issue because when the dog's on his back like this and the knee is in the air like this and i've chopped off the bottom of the femur and i've chopped off the top of the tibia i have no reference point so four or five degrees off on either of them could produce a skew with me but please god won't that's the biggest single risk is me getting it wrong in the alignment and that could happen but you're just gonna have to trust me on that there's no there's no other way it's quite rare for me to see a family so bonded and earthed with the animals you know they're right it's just amazing but your whole life revolves around them entirely yeah that's too unbelievable what unbelievable well i commend you okay all right big guy hey bobs kisses let's kiss [Music] all right big guy oh it's real sad be good adam don't be howling and keeping all the doggies awake into battle we go andrea's hoping the new knees will be the end of the young malamutes problems sad excited it's been a long time really waiting for this album's been suffering all time so it's a good thing it's a bad thing because i know he's going to be in pain but it'll be worth it oh i won it's 3 30 p.m and knowles urgently called to the consulting rooms harvey a four-year-old cocker spaniel has been brought in as an emergency okay hi buddy oh damn my goodness that's horrendous harvey's jaw has been fractured and he's already been given painkillers but the full extent of his injury is unclear all right buddy it's okay can you sit with him for a second what happened um i let him out last night to go to the loo before we went to bed and he came back 10 15 seconds later with his face smashed up well it looks horrific neither his owner nicola nor her sister kim has any idea how harvey was injured i thought originally he'd been attacked by a fox or something the whole front of his jaw has been ripped off all right buddy i know that that's really sore and there's no other damage on him nope nothing okay he must have gone harrying around and smashed into something or else maybe something was sticking out and and bashed into his jaw but it wouldn't have the only thing we thought about was the sprinkler was on the lawn is it strong enough to do that it is there was a sprinkler there was a sprinkler in the garden i suspect that's what it was well we got to fix him we can't leave him like that no yeah poor fella you poor fella yeah i know you poor fella poor man okay all right ladies give him a big cuddle please because james is going to take him through sweetie he'll be glad [Music] okay all right that's it all right mister don't don't get caught up in your drip [Music] there we go okay all right girls right thank you talk to you later good girl stay brave i'll talk to you tonight bye you're welcome noel sends harvey for x-rays and a ct scan straight away well i qualified as a vet when i was 22 and i'm 47. i've seen most of i think the injuries that can happen but in harvey's case running into something in the garden ripping his jaw off yeah i don't see that every day he's just such an intricate part of our family and it's not until something like this happens that you realise how precious they are harvey is extremely important to me and he's like my best friend when i get back from school he gets so excited and i don't really know how i'd be able to cope without him [Music] harvey a four-year-old cocker spaniel has been rushed in with a fractured jaw no one knows exactly what happened but x-rays reveal the end of his jaw has completely snapped off so you can see here where the front part of the jaw has been pulled down so obviously you had a right all impact poor fella and fixing it is not going to be easy harvey's fracture was particularly challenging it was right at the tip of his jaw there's not a lot of bone to work with so it wasn't an easy fracture to repair noel decides to try something he hasn't done before so this is exactly like putting braces on a human jaw what the wire did was give us a brace from the back right around the front to hook that jaw right up into position for park and i that was the first time we'd ever had the chance to use that technique i had read about it and just seemed perfect for this particular situation and you know it's very simple but it's cool it's very cool i'm just very carefully cutting the wire on each tooth and making sure not to cut the central wire because that's holding everything together feels nice and stable rock solid good that's pretty awesome yeah this rock solid porridge finishes off by coating the teeth and acrylic to cover the knots and strengthen the brace i think with how nasty that fracture was some people may have been tempted just to amputate that front bit of jaw because the fracture was so nasty but that would just leave the two back bits moving like that at least people you know that's an option it's a tough technique to pull off but it works that's a better option well yeah true at christmas the nurses see some of the year's more unusual cases a labrador had eaten a whole christmas pudding and quite posh pudding too the whole kennel room just smell of brandy it was that strong [Music] this dog had eaten all of the turkey eating a selection box without even taking the wrappers off cats come in quite often with tinsel in their abdomen most owners notice and be like it's like a shiny christmas booth just typical christmas stuff it's time for adam the malamute to have the first of his knee operations the ligaments around his knee are so weak knoll is using a custom-made implant [Music] this knee has been specifically made for items so it should fit perfect noel and his engineering team have spent years fine-tuning the implant design so it bends and flexes as naturally as possible okay this is a very complex and also a very simple implant so there's a peg here with a hole in the middle that goes on this goes through the hole this goes through the other side and that goes on there the knee can flex and extend but also you can twist the engineering may appear simple but fitting a brand new knee is highly specialized work it's a very artistic procedure it um demands that you think in three dimensions because you've got to get the angulation exactly right or you have a sticky in your sticky outing knee to hold the implant in place the ends of the tibia and femur bones are cut and filled with fast setting cement the cement holds the stem in the bone there and there until the bone physically grows onto the platform here okay we've got about four minutes from now to get everything right noel must insert and then perfectly align the new knee with adam's upper and lower leg before the cement sets hard don't talk to me right now i'm just dropping the implant into the cement now the stem goes down inside the cement inside the canal and the cement will bond the metal onto the bone okay the next moment is critical we're about to set the rotational alignment knoll can only gauge the implant's alignment with the angle of adam's foot by eye resting it against his stomach i'm looking at the implant i'm trying to get the front face of the implant exactly perpendicular to the foot i'm rocking the knee as if it was standing on the ground there we go that feels better hold it there chris that's it good and the alignment is as good as i can make it with my eyeballs just hope it's right good give me sporometric pds please it's 1 30 am and adam's first knee replacement has taken just over three hours to complete so the foot is in alignment with the knees in alignment with the hip that's what we want it looks nice it's not on any foot or an outy foot it's a straight foot good let's go to x-ray yeah okay so that looks alright no evidence of looseness that implant's fine all right good wake him up so i think we're on track to go home mid next week okay good night get some sleep finally noel's work for the day is over i finished last night half four i got up at half eight and i'm at a theater between one and two i am i must be mental i am mental i'm certifiably insane i've eaten three months boys i've eaten four you always have to go it's mid-morning and some tiny patients are causing a lot of excitement [Music] oh my god so cute it'd just be one one piece of dinner for you [Music] wendy and her daughter bertie have brought in binky their miniature dachshund and her three puppies pups are nearly six weeks old two of them are in to see the vet because one of them's got quite a bad front paw and one has like a slightly bad front ball because of the length of their body and the shortness of their legs the dashens can live without back legs but they can't live on one front leg so we were told worst case scenario it would be her being put to sleep and we were like no that can't happen and the lady with the three miniature yeah all right bye no mrs ward please hi guys how are you hello good thank you hello mom how are you hello puppies oh yo you're cute hello hello sausage hello i'm glad you call them sausages hello sausage too hello sausage three sausages sausages no what's happening what's this one called um that one's peggy because that's the dodgy leg and who's this one the one in the middle's biscuit oh that's kind of the one the dapple is amazing how amazing if you were in my life i'd get no work done it would be endless cuddle time wouldn't it so what's happening the one in the middle yeah peggy has got quite a strange front leg and then the dapple maisie has also got a bit of a strange front leg but we feel that one's kind of straightened out a little bit more since we saw our vets and then biscuit seems to be the only one of the three that's kind of normal all right sausages let's do a sausage race there we go number one off you go that one seems okay doesn't it yeah it's that one's that one is getting better yeah there you go oh nice piddle time hiddle time glad it wasn't on my chest yeah you holding so i'll clean the pig there we go right it's like being at home all right let's get off you go [Music] okay yes she's okay all right all right trouble here we go right not great okay well it's not the first or last time i will see this it's believed to to occur because the flexor tendons which do that are too tight relative to the growth of the bone okay and right now this little one is growing rapidly and getting longer and longer we saw if these tendons were to stay like that the limb will become deformed i'm very pleased you're here today and not here in two months time oh really because sometimes when paws carve over like that it's temporary like the dapple what i want you to do with mrs dapple is four times a day for 10 minutes put the wrist in your hands like that and extend it like so okay as if you're trying to do that okay you're stretching the flexor tendons okay okay now with this little one it's a whole other ball game because you're gonna need more stretching than that if peggy's poor is left untreated she might need surgery but as she's still young noel's hoping intensive physiotherapy will provide the cure now if i pull them over enough and i'm just going to show you once it will hurt she will not like that yeah and unfortunately she's going to have to go through that discomfort to get to where we need her to be okay what i'm going to propose to you is that we do this okay because frankly with the best will in the world you are not going to want to do that and that's sorry babe will we ever get her back though no no she's staying with me she could become fitzpatrick referrals house pet what fun what fun it's going to be weird now it's going to be weird at home but hey it's good it's really good news though he's thinking that it won't be surgery it will be just intensive physio that's brilliant that's really really good news really good news she's so so cute i put some little teddies in with her anyway just so that she because she [Music] [Music] it's been five days since adam the malamute's first knee implant keeping it out this way he's so excited to be out hold tight good lad here we go all right big guy good lad okay buddy he's doing very well there's no significant swelling which is the first big thing he's going to have to re-educate his muscles here because they're very weak so that's going to take a little bit of time but he's very comfortable when i move it good boy all right big guy right let's get you home good boy now look who's there look who's there looking at your skinny legs come on calm down there we go it'll be at least four weeks until noel can operate on adam's other leg but halfway through his treatment and for the first time in a year adam's future is looking brighter this is the place it's been quite emotional i didn't think it would affect me like it was but when i saw him when he come out well enough a bit now thinking about him when he dad cried dad that never cries never get sick sad never get stabbed i got the tape [Music] there you go some residents though will have to wait a bit longer before they can go home [Music] definitely unique we're not very good at this uh please be careful with that hey now can you help us are you eating the mouse are you eating the mouse is that your mom noel checks in on peggy the miniature dachshund puppy okay peggy let's have a look at you as peggy's growing so fast it's essential her poor uncurls as soon as possible if it doesn't noel may have to operate what we're thinking of doing is we're thinking of building a little splint here for peggy because she responds quite well actually to the the tendons being stretched but it's pulling back into a claw again so the plan is that this curve here at the end of the splint pushes her paw upwards and outwards where we want it yep bye bye all right be a good girl and i'll see you tomorrow get your pulse train yeah thanks a lot peggy though isn't so keen on the splint you're not helping peggy [Music] it came from the north pole that's why it's got i'm a 33 year old man and i buy presents for my cat and dog at christmas i've got stocking with bumper's name on it i even got one like a handmade crate with his name on the front of it to keep his ties in my parents seemed so disappointed it's been less than a week since harvey the cocker spaniel was brought in with a snapped jaw so harvey's made really good progress since we repaired his jaw you can see her on the x-ray picture where we put loops of wire around each of the teeth and that's pulled the the fracture right up into place there you can see where it's right back up in line so that's really good he's eating very well and uh he's happy yeah let's send him home just want him back and i want his crazy personality back as well maybe a little bit subdued just a little bit turned down hey buddy come on mom's here all right big guy harvey will have to put up with the brace and cone for at least another six weeks good boy there we go oh good boy so um if i if i lift his jaw there you will see his his braces underneath there you see that yeah so you can see it's covered in plastic what you've got to do is you've got to you got to just make sure that that stays relatively relatively clean and as long as it doesn't get dirty or crusty it's fine it's easy to manage he's a little hyper but as you say he's been through quite a lot he's quite high he's pretty mad anyway oh he's fantastic and he looks amazing home to our mad house and the girls will be just everyone will be so pleased to have him back home [Music] i don't like mince pies [Music] after seven days of intensive physiotherapy peggy's showing good progress oh that's tons better oh look at you yeah look at you that's awesome what do you think if we get her in the underwater treadmill yeah definitely she needs to keep strengthening so that'll be quite good yeah we get those tendons because they're moving much straighter now so we get a little power in him under water that would be good peg wouldn't it that would be good yes he would that afternoon peggy officially becomes the smallest dog ever treated on the hydrotherapy treadmill the water may only be five centimeters deep but for a six week old miniature dachshund it's an ocean we go [Music] it's 2 a.m and colin and andrea have rushed back to fitzpatrick's overnight in the hope of seeing knoll first thing in the morning it's only been two weeks since adam's knee surgery but he's in so much pain they're worried the operation has failed for the last couple of days he's been whimpering um and we don't know if it's his new knee or his other leg and i had to really help him or people he couldn't get up he was crying the implants are adam's last chance if they don't work nothing more can be done for him so it's scary it's scary because it'd be you know we could we could find out that it's not right it's moony which would be awful [Music] the next morning colin andrea and their daughter maddie brace themselves for a tough day ahead i think this is the um the crux of it now i think if um if this doesn't work i don't think he's got a future because i wouldn't want him to live the rest of his life in pain that won't be fair adam please come on we're going to go outside guys let's go this way can you walk the other side for me so tight or tighter on the lead please tighter and and walk down along yeah keep going keep going keep going keep going [Music] alrighty [Music] well here's what i think um if you chop a leg open and put a new joint in there's always going to be some perioperative discomfort that's inevitable but that pain is minor by comparison with how much pain is in on the other side noel thinks the problem is not the new implant but with the other leg which he's not due to operate on for another two weeks under normal circumstances i would never put put in a second joint within two weeks of the other joint i would just never do it i would never do it and you're putting all of the weight onto a knee i've operated on two weeks ago you're between a rock and a hard place [Music] the family have to decide whether adam can bear the pain for a few more weeks or if noel should rush ahead with the second knee transplant a big joint replacement is risky on its own but the risk is heightened if you're gonna have to do a second joint within a couple of weeks that's that's a big deal well i think i i need to give you a few minutes because you're going to need to have a think you know the risks scary i'm going to do it adam oh adam i don't think his leg's going to last another two weeks i don't know i don't think he's going to go and i think that's why he's been crying his lame's completely learned i think we should go for it knowing he is then adam okay what do you think i think we're gonna go for it now just understand we might fail so you need to know that it just seems the letter of two evils at least this way he stands a chance a bigger chance in my eyes because the alternative of leaving it longer i think the odds are not in our favor [Music] we're not going anymore it's late evening and adam's prepped for his second knee implant if this knee fails we're going to have massive problems just because we're halfway through now doesn't mean that we're going to succeed because there's very little options for adam here he would find it very difficult on three legs i thought we got to get on and fix it all right here we go [Music] over christmas someone will always be at fitzpatrick's looking after the patients i don't think anybody really minds working over christmas time because you are excited even though you're working it's just that exciting feeling i'd feel sad if my pets weren't at home on christmas day so it's our job to spoil it oh she's so cute i love her having cuddles with labradors and kittens and cats and things it's it's a nice way to spend the day adam the malamute has now had knee implants in both hind legs within two weeks and noel wants to check if he can support himself good boy that's it yeah oh good boy look at you getting up on your new knees good boy that's it that's it my friend that's it it's difficult for him because he's having to move his knees in an entirely new way and his muscles and ligaments are just not used to that but the good news is that he's making a real go of it good boy that's it and for colin and andrea watching adam begin to walk is possibly the best present they could wish for [Music] hello my little baby boy thank you misters we've adam will have to return every fortnight for physiotherapy but in the meantime you can enjoy some well-earned tlc malamute's going to be back look at you look at you he's looked with [Music] look at him and he looks so well and he's walking so well i did think we'd have to physically carry him out but he's brilliant absolutely fantastic yeah just look at him oh this is it [Music] a few weeks later adam's already looking stronger and steadier it's just brilliant that he's all he's arm and he's safe and his legs are healing slowly now it's about recovery and physiotherapy but um he's happy and he's well and he's not in pain and he's fantastic look at him i've got my boy back hey man cool come on lads ready [Music] good girl phoebe harvey the cocker spaniel is back for a checkup on his broken jaw [Music] it's now been two and a half months since his operation but until his jaw bones fully healed the energetic spaniel will have to continue wearing the wire brace and cone i'm a bit apprehensive if it's not fixed where do we go from here really so i'm a bit nervous of that but i'm being optimistic and it's going to be fixed and the nightmare is over i don't want to come in here today nicola will have to leave harvey to be x-rayed to determine if noel can finally remove the braces how's he doing hey mate i bet you're sick and tired of that now you want to get rid of it hopefully today's the day we get it off buddy yeah that's the plan we're gonna x-ray you first yep yep yep you know score yeah and then we've got to see if we can take it all off so i've just had i've just had a chat through the corner of communication he's heard all of that if it isn't if it isn't ready well that's what i'm nervous about today sure well i understand that eventually it'll heal it's just a matter of time okay i know he's desperate to run around and get get the cone of shame off [Laughter] okay in account from 10 bones backwards seven bones six bones five bones whether harvey is eating his christmas lunch with braces on or not three two three is definitely when i lost will only be decided after the x-ray got his x-rays he's healing but it's whether to make the jump to take it off or not yeah he's good let's go yep take it off okay buff look at that look at that solid as a rock now solid as a jawbone right you can breathe a sigh of relief with harvey signed off the family can now relax and enjoy christmas it's fantastic having him back and normal and he's back to his old self he still runs at 200 miles an hour into the back garden he hasn't changed at all i'm really looking forward to christmas with harvey because when we'll all be in the lounge and we'll be opening our presents and stuff he can sit with us which will be fun so it'll be a lovely christmas he'll be a big part after 17 days of physiotherapy peggy is also ready to go home and for her owners wendy and bertie it's not a moment too soon the other two puppies are gone it was really really hard and bertie and i had a sob in the card and we just sat there and stopped [Laughter] we've hung on to the fact that we're getting we're getting one back again you're so excited really excited yeah i can't wait hey bubs it's time it's time now hi fingers crossed she's not more fond of knowledge than us no hi everybody hi everybody i've got bigger much bigger hello babies oh you're so big pop her down see her walk she's walking great okay how much better is that come on come on then oh just look at that yeah and what's going to happen with peggy are you going to keep her or she going to go bertie and i would like to keep her well i would be delighted for you to keep her we could have visiting rights take care we shall miss you bye-bye bye thank you take care bye bye we shall miss her she's a sweet sweet little dog as christmas draws near a very special visitor arrives at fitzpatrick referrals rita fitzpatrick has flown in from ireland now everybody this is my mom hello she's just checking how the surgery is going you're done i'm next [Laughter] can you operate on her neck but she needs to fold a full job what exactly is the full job the full monty so this is lena isn't she beautiful gosh hello goodness look at the size of it about three days ago yeah lena was paralyzed and we replaced one of the discs in her neck my goodness and now she's able to walk again yeah you're very lucky yeah this is my messy office oh now this is where i spend half my life yeah a bit of a mess isn't it it sure is have you no one to tidy it i won't let anybody tidy it because you know where everything is yeah i don't know where everything is there's a picture of you over there receiving my award do you remember that yeah i don't know that's when i got the award in dublin that i gave to you oh yeah oh that's very nice yeah i i want him to be happy you have only the one life it doesn't matter if you're not happy enough what good is it your dad looked well he did look well his father was very sad workaholic work work work work so no one's the same i think he wouldn't he would like to have a wife but he hasn't time for one thank you happy christmas mum
