hi there everyone my name is ollie i'm a final year medical student at the university of warwick welcome back to the channel this is another one in the medical interview series of videos today we're going to be talking about the big daddy of all topics good medical practice what is it why does it matter who does it apply to why should you care you're not a doctor you're applying for medical school essentially good medical practice is a set of professional principles and guidelines that set out the behaviors that are expected of all doctors or at least what a good doctor should aspire to do it's written and published by the general medical council or the gmc which is the regulatory body in charge of maintaining the register of doctors in the uk that is who is allowed to practice medicine and who isn't and obviously that makes them responsible for dealing with doctors who do not follow the principles set out in good medical practice as with most things i feel like i cover on this channel you do not need to know the details for your interview however having a very basic understanding of what these principles are and more importantly why we have them at all that's going to be the key talking point and it can actually be helpful for answering many many different types of interview question it's split into four domains and we'll just briefly skim through each one trying to apply the themes of each domain to a medical case or something that might happen to you when you're a junior doctor on the wards because after all all of these are intended as guidelines of professional practice they're not just of academic interest things that you'll read once and then never do in real life these are your guidelines for surviving day-to-day as a doctor and they're supposed to help you in the real world not just get you through your sjt exam at the end of medical school so domain one is knowledge skills and performance and this domain revolves around making sure that obviously your clinical and academic knowledge is up to scratch and that your clinical skills as in the procedures and the things that you would normally do like taking blood bandaging wounds maybe doing minor operations if that's within your scope of practice essentially that you're performing at the level you're supposed to be performing at to keep patients safe this is one of the elements that can make healthcare work slightly unusual when certainly compared to other industries in that there is actually a requirement to make sure that your skills are kept up to date there is an obligation to demonstrate that you're maintaining competency over time in medical school for example that means passing all your exams at the time in which you're expected to pass them and actually this principle continues all the way through medical training until you become a consultant you'll still be taking exams because this represents you building more and more knowledge until you complete training and become a consultant not only that but you actually as a doctor have to get revalidated every five years because this demonstrates to the gmc that you are still fit to practice this is obviously to stop people qualifying as doctors and then just slowly forgetting everything they've learned because ultimately doctors especially more senior doctors are in charge of patient care and if the doctor doesn't know something in an emergency or gives them the wrong drug then it's the patient that ultimately is gonna suffer domain two safety and quality now this section obviously is about safety although specifically what it's more referring to is taking active part in the processes that operate within the nhs and within healthcare that keep patients safe so typically as an example this means taking part in what are called audits which are regular reviews of how staff are performing or doing things within a particular department within a particular theme so for example let's say you're a junior doctor working on the surgical ward you might decide to do an audit of post-surgical infection rates looking at how many people develop infections after they come into hospital have an operation and while they're recovering they develop an infection obviously that's not a good thing and it's something we want to minimize so you might look at infection rates in this surgical unit following gallbladder removal surgery over a period of three months and once you've got your first set of data you can think about ways to bring that number down so you might do things like giving everyone antibiotics before their surgery or if they're already doing that giving them a different antibiotic instead or limiting the number of people that are allowed into a surgical theatre at once basically it's just meant to be this ongoing process of improvement to try and keep patients more and more safe and receive better and better outcomes now the other element to this is not withholding information and taking an active part in any inquiries that take place as well as being watchful for problems that occur in the workplace and acting properly when they do so this means things like reporting properly whenever someone has an adverse drug reaction or filling out critical incident forms you might have heard of a datex checking all of the equipment that you're using is safe and in date etc etc and lastly the other thing to consider is your own health has to be considered because if you're interacting with patients you're a potential risk and a potential source of danger to patients so this means things like being registered with a gp while you're working as a doctor that isn't part of your family that you're immunized against the communicable diseases that you're supposed to be things like hepatitis tuberculosis for example and of course that your judgment is not likely to be impaired by anything that you're doing so no turning up to work under the influence of drugs or alcohol not attempting a surgical operation when you're too tired in reality to do so maintaining vigilance over your own health and awareness over your own health and judgment we're the responsibility to work well with patients and your colleagues and colleagues is quite a broad brush stroke there but we'll go into that in a minute with regards to patients this means elements like listening to them properly being polite gentle considerate when speaking to them providing them with all the information that they would wish to know in a format that is suitable for them we don't hide anything from patients if they specifically ask about something we have to tell them often we will have to imagine what a reasonable person in their situation might want to know and we have to address any barriers that exist in the way of communication which means things like addressing different learning needs learning differences or different cultural barriers as well because even though these things can often be in the way the patient still needs to have that information and they need to benefit from having it on the other side this domain also includes teaching training and supporting our medical and non-medical colleagues did you know for example that all doctors regardless of what level you are who you are have a professional obligation to contribute to the teaching and training of medical students doctors and allied health care professionals i know a lot of people don't like teaching and it's not for everyone however personally i think that's probably more a consequence of medical schools not training doctors in teaching methods not teaching people how to teach so that means that not that many people are confident teachers and therefore they're not passionate or interested but in general as far as you and me are concerned this is going to mean teaching medical students when we're junior doctors because again they're the next generation of doctors that need to come through and be good for patients when you become a registrar you need to teach the junior doctors when you're a consultant need to teach the registrars and everyone below you in seniority and so on and again this doesn't just mean medical students and doctors this can be healthcare professionals from any other stream nurses nursing students paramedics psychologists pas everyone else involved in patient care if they can stand to benefit from some teaching or equally if you stand to learn something from them engaging with that process can only be good for patients and therefore people have a duty to do it and engage then lastly domain four is maintaining trust this last one is a bit simpler and it is as simple as maintaining trust in the medical profession because crucially what you need as a medical profession is the public to trust them to tell them the things that they need to know in order to provide care and keep them likely to seek help and seek medical assistance when things go wrong more specifically this might mean things like not pursuing improper romantic relationships with patients treating patients according purely to their clinical need without any form of discrimination or judgment on our part and not allowing your personal beliefs to come in the way of giving someone proper medical attention or indeed to express your personal beliefs in such a way that it would cause a patient distress doctors also need to act with honesty and integrity whenever they're in medical practice and usually beyond that as well this includes things like giving your full name and gmc registration details if a patient or another staff member asks for them which might happen if you make a mistake or an investigation needs to be made these things happen but again you have a duty to engage with that process you can't hide anything you have a duty to be honest up front and not leave out any relevant information so those are the four main domains in good medical practice guys but in general if you take nothing else away good medical practice outlines the professional behaviors expected of all practicing doctors and they serve as the
